Job overview
You’ll support patients receiving community-based care, helping them maintain wellbeing and independence through safe, compassionate nursing support. Based within Integrated Community Services, you’ll work closely with colleagues across the wider healthcare team to deliver high-quality care and positive outcomes for patients.
This role is ideal for a registered nurse who enjoys building relationships, making clinical decisions, and supporting others to develop. You’ll have the opportunity to work both independently and as part of a team, contributing to a caring environment where patient safety, dignity and respect are at the heart of everything you do
Main duties of the job
- Assess, plan, deliver and evaluate nursing care for a group of patients
- Provide safe, person-centred care and respond to patients’ changing needs
- Support and guide junior staff, learners and students during their development
- Maintain accurate patient records and contribute to clinical governance activities
- Help coordinate care and support the running of the service during a shift when required
- Registered Nurse qualification (RGN/RN or equivalent)
- Recent experience working in a nursing role
- Strong communication and teamwork skills
- Ability to work independently and make sound clinical decisions
- Commitment to learning, development and delivering evidence-based care
