Job Title: Registered Nurse Childrens Continuing Care (Community)

About the Trust Hampshire and Isle of Wight Healthcare NHS Foundation Trust provides a wide range of community, mental health and learning disability services for people across Hampshire and the Isle of Wight.

The Trust provides most of the NHS services that are delivered away from the big acute hospitals in peoples homes, communities and in local hospitals and clinics.

This flexible role as a Registered Childrens Community Nurse for NHS Professionals offers a unique blend of professional fulfilment and adaptable scheduling. This position allows nurses to provide vital healthcare services within the community, making a significant impact on patients' lives while having the freedom to choose their working hours. The flexibility of a bank role enables community nurses to balance their professional duties with personal commitments, fostering a healthy work-life balance. Main duties of the job

What you can add:

  • A car and valid UK driving license
  • As a registered nurse to always work within and uphold the NMC Code.
  • To assist in the delivery of care activities in the child/young persons own home and other community settings.
  • Staff will work unsupported following assessment with the support of the Clinical Case Manager. Has responsibility for the induction and development of new care staff, under the direction of the Clinical Case Manager.
  • To be flexible to work flexible hours, 4/5 week rota pattern including weekends and Bank Holidays.
  • Undertake clinical tasks as delegated by the Clinical Case Manager.
  • Responsible for working with children/young people who are dependent on others for meeting some or all of their health and wellbeing needs.
  • Ensure the care that is delivered to the children on the caseload is of a consistently high standard
  • Responsible for discussing the childs/young persons support plan with the qualified nursing team, ensuring understanding of their role in providing care to meet the needs, including an understanding of the Safeguarding of Children.
  • Responsible for delivering care in accordance with relevant care plans, escalation plans, Standard Operating Procedures and guidance.

The role will involve, but not be limited to you performing the above.
