Main duties of the job

IOS provides short-term, intensive, psychosocial support to people being discharged from AWP Intensive Services to their GP. As a Recovery Navigator, you will provide support that is: Short-term (up to 6 weeks) Intensive (2-3x per week) Person-centred 1:1 Outreach Who does IOS support? Individuals being discharged from AWP Intensive Services (IS) back to GP-led care Have remaining acute symptoms of risk or distress Would benefit from an extension of intensive support, but no longer require clinical intervention

Job responsibilities

Hold a small caseload of up to 6 clients Work collaboratively with individuals to understand their experiences, strengths, challenges, and risks Work in collaboration with intensive teams in AWP Contribute to the development of client formulations Create client support plans, and safety plans Deliver psychoeducation and skills-focused interventions Deliver support which is focused on reducing: oSuicidality oself-harm oAcuity of mental health symptoms

Person specification

Experience

Essential

  • A minimum of 6 months-1 year experience working with people with mental health needs, including risk assessing and support planning
  • Experience of providing practical and emotional support to enable the client to take up the responsibilities of their own recovery.
  • Proven experience of working with people with multiple needs (e.g. mental health, homelessness, alcohol/drug dependency long term conditions)
  • Proven experience of carrying out needs and/or risk assessments and the support planning process
  • Experience of working with a number of individuals with competing needs and priorities

Desirable

  • Proven experience of mental health issues and/or services, either as a user, carer or supporter.
  • Experience being involved in processes with high-risk presentations, including risk management, safety planning, safeguarding
  • Experience delivering support to acute populations within crisis, admission or intensive community settings, including managing complexity, co-occurring needs and heightened risk.
  • Lived experience of mental health issues or services
