Rapid Response/Urgent Crisis Response Clinician Band 6
Work as part of a team of clinicians, therapist’s rehabilitation assistants and Community Geriatricians, prioritising avoidance of unnecessary hospital admission. Assessment and treatment of patients during a crisis, identifying complex and specialist need and developing, implementing and evaluating programmes of care. In addition, facilitating early hospital discharge for patients back to their own home. To Support and promote the on-going development of all care delivery within the patients own home, enhancing the quality of care delivered. As a clinician working with a high level of autonomy, within agreed guidelines, managing undifferentiated and un-diagnosed problems using advanced clinical skills by incorporating physical assessment and diagnosis

Main duties of the job
Working as part of the Rapid Response team, the post-holder will provide assessment and treatment of patients, within their own home, during a crisis. The post holder will identify complex and specialist needs and will develop, implement and evaluate programmes of care.
Working as a practitioner with a high degree of autonomy, the post-holder will manage undifferentiated and undiagnosed problems using advanced clinical skills by incorporating physical assessment and diagnosis and be supported to do this by senior members of the MDT.
The post holder will also identify patients suitable for virtual ward monitoring and be proficient in fitting the appropriate technology following training and support from the team and will be responsible in assessing unwell patients at home on request of the virtual ward hub.
