- Contribute to the innovative approach of the Integrated Children & Families Service across Oldham, with flexibility to work from various bases based on community needs

Main duties of the job
Helping Children and Families Thrive: Your Key Contributions
As a dedicated member of our team, you'll be instrumental in:
- Empowering Healthy Child Programmes: Supporting the delivery of the Healthy Child Programme, a vital initiative that promotes the well-being of children and families in our community
- Tailoring Health Support: Assessing the unique health needs of Looked After children and young people, and providing targeted interventions to help them thrive
- Health Interventions with a Personal Touch: Delivering holistic health interventions to children and young people who need them most, ensuring they receive the care and support they deserve
- Conducting Health and Well-being Reviews: Visiting homes, family hubs, and community settings to conduct comprehensive health and well-being reviews, helping families achieve their goals and work towards positive outcomes aligned with the Healthy Child Programme

- Good understanding of how to work.

- Ability to communicate at all levels both written and oral
