We are looking for an experienced and enthusiastic Practice Nurse to join our friendly and supportive team.

The role is for 30-37.5 hours per week (flexible for the right candidate). Previous General Practice experience is essential, with skills in cervical screening and chronic disease management. Chronic Disease Management would be an advantage but Older Adult Vaccinations, Wound Management and an understanding of QAIF are essential requirements for this role.

Desirable skills include Independent Prescribing Minor Illness Management

Duties and Responsibilities:

Assist in and perform routine tasks related to patient care as directed by Nurse Lead and GPs

Cytology

Venepuncture

Adult Vaccinations

Requesting pathology tests, for example urine culture, swabs

LTC management including Diabetes, Coronary Heart Disease, Hypertension, Asthma, CVA, COPD

Ordering of vaccinations to maintain stock levels

Ability to deliver COVID Vaccinations

Following agreed clinical protocols with referral from GPs as appropriate

Awareness of infection control and requirement to take part in infection control audit

Participation in administration systems in the Practice (EMIS and Docman)

At all times there will be a need to maintain accurate records

A duty to advise GPs of potential problems or errors within a range of assigned tasks

Attend and participate in any Nurse and Practice meetings when required

Any other delegated duties appropriate to the post

We are a small friendly practice of 5500 patients in the ex-mining Valleys community of Abertillery.

Cwm Calon is a medium-sized training practice in the Blaenau Gwent area, led by a young and dynamic team of Partners.

We have recently been awarded the contract for Aberbeeg Medical Practice and are excited to expand our team across both sites and looking for staff at this site.

Job responsibilities

The successful candidate will become part of a well-established nursing team, working collaboratively with the wider multidisciplinary team while demonstrating the ability to work independently when required.

  • The successful candidate must have prior experience working as a Practice Nurse within General Practice and be competent across core practice nursing duties.
  • Experience in cervical screening and chronic disease management is essential

Qualifications

Essential

  • GCSE Grade 1 to C in English and Maths
  • Nursing degree and NMC Registered Nurse
  • Management of chronic diseases
  • Cervical Cytology
  • Applicants should ideally already have the essential skills and experience required of a Practice Nurse. However, further training and support will be provided, where required, to enable the successful candidate to fully undertake the role and to develop or enhance existing skills.
