
Job summary
We are looking for an experienced, enthusiastic, friendly and professional Practice Nurse with Interest in Diabetes to join our team at Pontesbury & Worthen Medical Practice on a permanent contract basis. Applicants must have experience of working in a GP Practice.
The successful applicant would be working alongside our current nursing team, and be responsible for the delivery of Diabetes practice nursing services to our patient population.
Main duties of the job
Your main responsibilities will be the routine evidence based care of our diabetic population, including annual reviews incorporating the local prescribing guidelines as well as following up patients with more complex diabetic needs. The ideal candidate would have current Primary Care experience and would be a non-medical prescriber but this is not essential.

- The role of the diabetes nurse is to promote the health and wellbeing of people with diabetes, to encourage compliance with medication through informed self-management.
- The post holder with work with and liaise on a regular basis with the Practice GP diabetic leads and nursing team to manage patients with diabetes and to assist in the development of the diabetes service.
- Identify clinical risks and take appropriate action.
- Provide a pathway of diabetes nursing care for the patient, referring to secondary care, multidisciplinary teams and voluntary services as required.

Experience
Essential
- Experience of working in a GP Practice
- Experience of EMIS or other practice clinical system
- Excellent communication skills (written and oral) and interpersonal skills
- Strong IT skills and competent in the use of Office and Outlook Effective time management (planning and organising)
- Ability to work as a team member and autonomously
- Problem-solving and analytical skills
- Ability to follow clinical policy and procedure
Desirable
- Experience with audit
- Experience with clinical risk management
Qualifications
Essential
- Advanced clinical and diabetic knowledge
- Current NMC registration with active pin
- Relevant Nursing Degree
- Diabetes Nurse with General Practice experience
Desirable
- Postgraduate diploma or degree (Primary Care)
