Greenhill Medical Centre & Clase Surgery is seeking a Practice Nurse / Chronic Disease Nurse to join our friendly and supportive team.
This is a 15-hour per week role focused on chronic disease management, including diabetes, respiratory and cardiovascular conditions, alongside general practice nursing duties.
We are looking for an enthusiastic and compassionate NMC-registered nurse who is committed to delivering high-quality, patient-centred care.

Main duties of the job
The Practice Nurse / Chronic Disease Nurse will work as part of the multidisciplinary team across Greenhill Medical Centre & Clase Surgery, reporting to the GP Partners and Practice Manager.
The role will involve weekly chronic disease clinics, including diabetes, asthma, COPD, hypertension and cardiovascular disease, alongside routine practice nursing duties such as health checks, vaccinations, wound care, ECGs, observations and health promotion.
The post-holder will maintain accurate clinical records, support patient recalls, contribute to audits and quality improvement, and work closely with GPs and the wider practice team to provide safe, high-quality patient-centred care.
Training and development opportunities will be supported where required, subject to the candidate's experience and role requirements.

Essential
- Post-registration clinical nursing experience.
- Experience of assessing and caring for patients with long-term conditions.
- Experience of working effectively within a multidisciplinary team.
- Experience of maintaining accurate clinical records.
- Experience using computerised systems.
Desirable
- Previous experience in general practice or primary care.
- Experience of chronic disease management, particularly diabetes, asthma/COPD and cardiovascular disease.
- Experience in wound care, ECGs, venepuncture and immunisations.
- Experience using EMIS Web.
Qualifications
Essential
- Registered Nurse with current NMC registration.
- Appropriate nursing qualification.
- Evidence of continuing professional development.
- Ability to work within professional scope of practice.
Desirable
- Qualification or additional training in chronic disease management.
- Diabetes or respiratory disease training.
- Cervical screening qualification.
- Immunisation and vaccination training.
- Minor illness qualification/training.
