Job summary

We are looking for an experienced, enthusiastic, flexible, committed and motivated Practice Nurse to join our team at Y Felinheli and become involved in all aspects of the Practice.

Excellent communication, interpersonal and organisational skills are essential, as are computer literacy and the ability to work as part of a team.

Experience in Primary Care Nursing duties including chronic disease management is highly desirable, as is the ability to converse in Welsh.

The successful candidate will be able to work well under pressure and using his/her own initiative, and will be required to have a Registered General Nurse qualification and work within the NMC code of professional conduct at all times

Main duties of the job

Main Duties and responsibilities: To be able to hold sessions for patients without seeing a GP first

  • To provide a Nursing diagnoses, discharge or refer independently
  • Encourage the development of management and prescribing protocols
  • To be cost effective
  • Autonomously manage chronic disease
  • To participate in nursing needs assessment and discharge nursing responsibilities to patients when necessary
  • To present all alternative choice of care to patients

About us

Felinheli and Menai Bridge Surgery has been providing medical care to local patients since 1967.

We are a 3 Partner, 7000 patient Practice. We also have 2 salaried GPs and 1 ANP working at the practice along with 2 practice nurses and 2 healthcare assistants.

The Practice is based on two sites (5 miles apart) either side of the Menai Straits that separates Anglesey from Gwynedd. The main surgery is in Felinheli, Gwynedd and is a dispensing site. The other site is in Menai Bridge, Anglesey.

We are located in a beautiful part of North Wales with its stunning coastlines and fabulous Snowdonia / Eryri National Park.

Job responsibilities

Job Summary:To assist medical personnel in the care of Practice patients to include treatment, preventative care, screening and patient education.

  • Responsible for attending to patients who wish to be seen by a health care professional; provide a Nursing diagnoses and treating patients presenting with minor illness seeing and advising people in respect of their continuing medical and nursing needs.
  • Patients will have booked appointments at ten-minute intervals. There is a requirement to see extra or emergency patients.
  • The Nurse will need to prioritise and triage the needs of patients accordingly making any necessary referrals for investigations in the appropriate manner.
  • Patients with problems needing referral to secondary care should be discussed with the registered GP before making such referral.

Job Responsibilities:

Providing assessment, screening and treatment services and health education advice

Organise and co-ordinate the provision of Nursing services for the Practice to include the delegation of duties to other members of the Nursing team as appropriate.

Provide Nursing treatments to patients in participation with general practitioners or independently to agreed protocols.

Provide general and specific health screenings to the Practice patients (within agreed protocols) with referral to General Practitioners as necessary.

Advise patients on general health care and minor ailments with referral to GPs as necessary.

Diagnosing and treating patients presenting with minor illness

  • Triage and treat patients wishing to see a health care professional making any necessary referrals to other members of the primary health care team.
  • Ensure clinical practice is safe and effective and remains within boundaries of competence and to acknowledge limitations.
  • Advise patients on general health care and minor ailments with referral to other members of the primary and secondary health care team as necessary.

Supplies and equipment Treatment room and other areas

Restocking and maintenance of clinical areas and consulting rooms in liaison with the Health Care Assistant.

Pathological specimens and investigatory procedures

Following appropriate training, undertake the collection, storage and despatch of pathological specimens including intravenous blood samples, swabs, smears etc and ensure the cleanliness of any equipment so used.

Perform any investigatory procedures requested by the GPs.

Administrative and professional responsibilities

Participate in the administrative and professional responsibilities of the Practice team.

Ensure accurate notes of all consultations and treatments are recorded in the patients notes on the clinical computer system as appropriate.

Ensure accurate completion of all necessary documentation associated with patient health care and registration with the practice.

Ensure collection and maintenance of statistical information required for regular and ad hoc reports.

Attend and participate in Practice meetings as required.

Assist in the formulation of Practice philosophy, strategy and policy and develop appropriate protocols.

Student/staff education and training

Participate in the education and training of students of all disciplines and the induction of all members of practice staff with special responsibility for Nursing staff.

As well as the Nursing team there is a need to work closely with reception, office and dispensary staff to ensure the smooth running of the Practice, reporting any problems encountered to the relevant person and ensuring everyone is aware of the different roles within the Nursing team.

  • Maintain effective liaison with other agencies and staff concerned with patient care and with all other disciplines within the practice, with appropriate regard to confidentiality.
  • There is also the need to establish and maintain good liaison with other surgeries and agencies including secondary care

Training, professional and personal development

  • Maintain continued education by attendance at courses and study days as deemed useful or necessary for professional development.
  • Training requirements will be monitored by annual appraisal and will be in accordance with Practice requirements. Personal development will be encouraged and supported by the Practice. It is the individuals responsibility to remain up to date with recent developments.
  • Participate in the education and training of students of all disciplines and the introduction of all members of the practice staff where appropriate
  • Develop and maintain an in-house nurse-training programme reporting to the Practice Manager.
  • Maintain continued education by attendance at courses and study days as deemed useful or necessary for professional development.
  • If it is necessary to expand the role to include additional responsibilities, full training will be given.
  • Develop and maintain a Personal Learning Plan.

Manage, supervise, co-ordinate and guide the nursing team

  • To help manage the Nursing service in participation with the Practice Manager ensuring relevant information is disseminated to and from the nursing staff
  • Ensure adequate Nursing cover is provided while within the Practice guidelines, keeping the Practice Manager informed of any problems
  • Develop and maintain Practice policies and protocols for all Nursing activity
  • To be the infection control lead in the Practice ensure all staff are aware of current procedures
  • Ensure agreed Practice audits are carried out
  • Ensure the Practice Manager informed of any matters arising or problems / potential problems
