The Perinatal Community Care Co-ordinator is a Band 6 clinician working within the Leeds Perinatal Mental Health Service, providing specialist assessment, treatment, care co-ordination and support to women experiencing acute moderate to severe perinatal mental health difficulties. The post holder acts as an autonomous practitioner managing a complex caseload of mothers and infants, supporting women and their families in the community, facilitating discharge from inpatient services, and helping to prevent hospital admission where possible. The role involves multi-agency working with health, social care and third-sector partners, safeguarding children, delivering evidence-based interventions, participating in regional duty systems, supervising junior staff and students, and contributing to service development across Leeds and the wider Yorkshire and Humber region. The role includes supporting the Duty and Urgent Assessment Team (DUAT) by monitoring the duty inbox, reviewing referrals, undertaking telephone triages and determining suitability for the service prior to assessment.

This vacancy is for 2 part-time positions (22.5 hrs p/w). One permanent and one 6m fixed term post.

Main duties of the job

The successful candidate will provide specialist perinatal mental health assessment, care co-ordination and treatment for women experiencing acute moderate to severe mental health difficulties during pregnancy and the postnatal period. They will manage a complex caseload, undertake risk and safeguarding assessments, develop and review care plans, and work closely with mothers, infants, families and partner agencies. The post holder will deliver evidence-based interventions, support recovery and parent-infant relationships, participate in regional duty systems, chair multi-agency meetings, provide supervision and mentorship to staff and students, and contribute to service development, quality improvement and safeguarding practice across the service.
