Proposed salary: Band 5.1 - 5.12 on the ABC pay scale, which is equivalent to £28,888.23 - £35,918.88 per annum dependent on experience (pro rata)
Hours of work: 15-20 hours Part-time/ Job Share. Occasional Saturday mornings may be required
Tenure: fixed term contract initially until 31st March 2027
Base: Leacroft Medical Practice with travel to other Crawley GP practices and ABC offices in Horley and Crawley
Main duties of the job
The ADHD Primary Care Connect service is a new, GP led pilot in partnership with Sussex Partnership Foundation Trust, one of the largest mental health trusts in the country.The service, which is being piloted in Crawley, will accept referrals from Crawley GP practices for patients with suspected ADHD and will offer initial assessment, diagnosis and treatment, including holistic patient support.
As a Patient Services Coordinator for the ADHD Primary Care Connect Service, you will play a crucial role in supporting the efficient operation and development of the service.Working closely with patients, GPs, local practices and ABC colleagues, you will be responsible for coordinating and optimising service delivery and ensuring a seamless experience for our patients and clinicians.There will be a strong focus on partnership working and additional opportunities for peer support for those in similar roles in other GP Federations.You must be comfortable, and confident, supporting patients to engage with the service by adopting a solution focussed, patient friendly approach and methodology.

Job responsibilities
Primary duties and areas of responsibility
Patient Support & Coordination
- Act as primary point of contact for patients, carers and family members via phone, email and in person.
- Coordinate, receive and process referrals in accordance with service policy.
- Guide patients by offering support to complete questionnaires, forms and assessment paperwork, offering clear guidance in an accessible and supportive manner.
- Guide patients through the ADHD assessment and treatment pathway, explaining processes, timelines and next steps.
- Navigate appointments with patients, including booking, confirming, rescheduling and reminding patients of upcoming appointments.
- Monitor attendance and follow up with patients who miss or cancel appointments.
- Maintain a short-notice waiting list in order to optimise appointment utilisation in the event of patient cancellations
- Maintain a professional, empathetic and non-judgmental approach in all patient interactions.
Clinical Team Administrative Support
- Service coordination including administrative support to clinicians, diary management and rota coordination.
- Coordination of clinics including the preparation and management of clinic appointment lists.
- Support the management of clinical correspondence, reports and documentation.
- Assist in the coordination of multidisciplinary team MDT meetings, including scheduling and preparation of agendas and minutes where required.
- Track workflow and ensure clinicians have the information and documentation needed for appointments.
Data Management & Governance
- Accurately input and update patient information on electronic patient record systems.
- Ensure all data is handled in line with confidentiality, consent and information governance policies (e.g. GDPR).
- Maintain organised digital and paper filing systems.
- Support service audits and data collection where required.
Service Support & Improvement
- Identify gaps or delays in administrative processes and suggest improvements.
- Support service development projects and quality improvement initiatives.
- Contribute to a positive, inclusive and supportive service culture.
Person specification
Experience
Essential
- Proven experience in an administrative, coordination or customer-facing role (healthcare or mental health desirable).
- Experience working as part of a team and working independently
Desirable
- Knowledge of GP clinical systems
- Experience working in mental health, neurodevelopmental or ADHD services.
- Understanding of ADHD and associated challenges across the lifespan.
Qualifications
Essential
- Educated to NVQ 3 level in a relevant subject or equivalent level of qualification or significant previous proven experience

- Demonstrable commitment to professional and personal development
Skills and Knowledge

- Excellent communication skills both verbal and written
- Strong organisational and time management skills
- Confidence in supporting patients to complete forms and understand process
- Experience managing rotas or clinical schedules.
