Job summary

The homeless healthcare team at Urban Village Medical Practice is recruiting an enthusiastic non-clinical team member to support the administrative activities of the service and patient engagement.

This role entails a combination of administrative support to the team as well as direct engagement with our patients throughout their healthcare journey.

We work in partnership with health and homelessness services across the city to reduce the severe health inequalities faced by people experiencing homelessness.

The successful candidate will be provided with a full induction and training.

Main duties of the job

  • Administrative support to the operational lead and homeless healthcare team.
  • Direct support to patients to help them to navigate accessing health care, signposting and referral to other agencies as required.
  • Working in partnership with other services to improve patient outcomes.
  • Supporting the general practice team to improve access for homeless patients.
  • Supporting with data collation and patient follow up.
  • There will be opportunities to engage directly with our patients both in the practice and in outreach settings, for example, assisting with new patient registration, supporting the outreach team and helping patients to navigate their health needs.
  • Safeguarding is everyone's responsibility and is a fundamental aspect of working within our homeless healthcare service.

About us

Urban Village Medical Practice based in Ancoats is a busy City Centre GP Practice with over 14,000 registered patients. Urban Village Medical Practice has recently been recognised by CQC as outstanding.

We have delivered a dedicated homeless healthcare service for over twenty years. We have approximately 850 registered patients under the homeless service.

The homeless healthcare team delivers daily GP & nurse clinics from the practice, a hospital in-reach service supporting homeless inpatients at Manchester Royal Infirmary & nurse led outreach to hostels and day centres across the city.

The successful candidate will join a multi-disciplinary team of clinical and non-clinical staff who are dedicated to improving the health and lives of people experiencing homelessness in Manchester.

The team prides itself on being supportive, professional and approachable. We hold regular team meetings to discuss complex scenarios, as well as informal discussions daily within the office. This provides a unique learning environment and supports the wellbeing of all of our team members to ensure that we can deliver an outstanding service to patients.

Job responsibilities

We are happy to discuss arrangements as to part time, full time and flexible working. Our core service hours are Monday - Friday 8am - 4pm.

  • assisting with new patient registrations and supporting patients to access the service.
  • handling requests for information from patients and external organisations with support from service leads.
  • working with hospitals and outpatient care providers to enable our patients to access healthcare.
  • managing post received for patients using the practice as their care of address.
  • supporting with data collation and patient follow up.
  • participating in multidisciplinary team meetings.
  • responding to enquiries from patients and external organisations, providing support and signposting to other agencies where appropriate.
  • supporting the outreach team to engage patients to attend appointments at the practice and elsewhere.
  • ability and willingness to support our nurse led outreach by driving our clinical van is beneficial but not essential.
  • due to the nature of our service an understanding of safeguarding responsibilities is essential, training will be provided to the successful candidate.
  • contributing to ongoing development of the homeless healthcare service working alongside statutory, third sector and voluntary services.
