Job summary

Be at the heart of transforming patient care as a Patient Flow Coordinator (PFC) within the Transfer of Care Hub. In this fast-paced and rewarding role, you'll play a critical part in helping patients leave hospital safely, sooner, and with the right support in place.

Working alongside a dynamic multidisciplinary team including nurses, social workers, and community partners you'll help drive seamless, person-centred discharge planning. You'll connect the dots across acute and community services, gathering key information, coordinating care, and ensuring every patient's journey out of hospital is smooth, safe, and tailored to their needs.

As a trusted link between wards, services, patients, and families, you'll combine compassion with coordination, supporting individuals through what can be a stressful time, while keeping patient flow moving across the wider health and care system. Your work will have a real impact, reduce delays, and ensure hospital beds are available for those who need them most.

Main duties of the job

The Patient Flow Coordinator (PFC) supports timely, safe, and effective discharge of patients from the acute hospital into community health and social care services. The role involves gathering and coordinating information from a range of sources to support patient-centred discharge planning, ensuring patients are transferred to the most appropriate care setting as efficiently as possible.

The PFC contributes to maintaining patient flow across the Portsmouth and South East Hampshire system while acting as an advocate for patient safety, dignity and individual needs.

Support and maintain momentum in discharge planning in collaboration with the Integrated Discharge Service (IDS), ward multidisciplinary teams (MDT) and community partners

Act as a key link between the Transfer of Care Hub (ToCH) and acute/community wards

Gather, collate and interpret patient information from multiple systems and MDT sources to support discharge assessments

Facilitate effective communication between patients, families, hospital staff and community services regarding discharge plans

Provide knowledge and guidance on discharge processes and pathways

Identify, escalate, and assist in resolving discharge delays, risks, capacity issues and complex cases appropriately

Support arrangements to ensure all elements of discharge requirements are in place prior to patient transfer

Assist with responding to internal and external discharge-related enquiries
