Job summary

PMLs Collaborative Care Team (CCT) supports seventeen General Practices located within West Northamptonshire, providing proactive assessment and support to people with longterm and complex conditions and their Carers. We undertake holistic assessment within the home environment, identifying needs and priorities with patients and their carers to improve wellbeing, maintain independence with an overarching aim to prevent unplanned admissions to hospital.

The Team currently comprises two Registered Nurses (RNs), an Occupational Therapist (OT), two Assistant Practitioners (AP), and a Social Prescribing Link Worker. The CCT operates Monday to Friday, 08:30 to 16:30 excluding Bank Holidays

Main duties of the job

To provide efficient, accurate, and proactive administrative support that contributes to the effective operation of services. The role holder is responsible for managing routine and moderately complex administrative activities with minimal supervision, applying established procedures, sound judgement, and attention to detail. The role supports effective communication, robust information management, and consistent service delivery in line with company, team, and NHS standards across clinical and non-clinical areas.

Job responsibilities

Position: Patient and Service Administrator

Department/team: Collaborative Care Team (CCT)

Role: Administrator (L2)

Reports to: Collaborative Care Team Clinical Lead

Accountabilities

Service Coordination and Operational Support

Support the day-to-day coordination of the CCT service under the direction of the Clinical Lead. Act as a first point of contact for patients, carers and professionals, ensuring enquiries are handled efficiently and support is provided where needed. Assist with scheduling, appointments and service activities.

Team Liaison and Coordination

Work closely with colleagues, GP Practices, PCNs and other healthcare professionals to support coordinated, person-centred care. Share information appropriately, follow up actions and attend team meetings to help ensure patients receive timely support.

Service Administration

Provide administrative support to the CCT service, including managing correspondence, preparing documents and reports, maintaining filing systems, and supporting governance, performance and operational requirements.

Clinical System Administration

To develop expertise in the administrative functions of Clinical systems, specifically Systmone and EMIS, including problem solving, staff support, access, reporting and contingency.

Record Management and Compliance

Maintain accurate and up-to-date patient and service records in line with information governance, GDPR and confidentiality requirements. Support audits, monitoring activities, complaints and quality assurance processes to help ensure the service remains safe, effective and compliant.
