Job overview

An exciting opportunity has arisen within the Hospital Palliative Care Team at Queens Hospital Burton, for a Pathway Co-ordinator.

The post holder will be required to provide administrative support to the Hospital Palliative Care Team. They will need to work flexibly, alongside nursing staff, consultants and other teams when required.

We are looking for an enthusiastic and motivated team player with excellent communication & problem-solving skills. You will require high levels of tact and diplomacy with the ability to prioritise your own workload effectively to enable day to day management.

Main duties of the job

  • You will act as the main point of contact for patients and clinical staff on behalf of the consultant and hospital palliative care team, ensuring that all queries are answered with skill and sensitivity and in a courteous and professional manner.
  • Preparing the weekly multi-disciplinary team meetings.
  • Attending and recording minutes of team meetings and clinical multi-disciplinary team meetings.
  • Typing and dispatch of clinical correspondence within agreed timescales and quality standards.
  • Liaise effectively with wards, departments and other clinical staff within the hospital and external healthcare providers.
  • Administration and clerical duties including appointment scheduling,  answering telephone queries, managing referrals.
  • Gather relevant documentation to accompany referrals e.g. clinic outcomes, test results & discharge summaries.
  • Support with adhoc administrative duties
