Job summary

An exciting opportunity has arisen to join Ashgate Hospices Community Specialist Palliative Care Team as a Palliative Care Nurse. You will deliver compassionate, high-quality, person-centred palliative and end-of-life care to patients and the people important to them across North Derbyshire. Working within the community, you will undertake holistic assessments, develop and evaluate personalised care plans, support complex symptom management, and contribute to advance care planning in collaboration with the wider multidisciplinary team. This post has been created as part of the expansion of our Community Specialist Palliative Care Nursing Service from five to six days a week, improving continuity of care for patients and families, particularly over weekends. The role includes community visits, lone working, participation in clinical triage, and close partnership working with health and social care colleagues. We are looking for an experienced registered nurse who is passionate about delivering outstanding palliative care and keen to develop their specialist knowledge and leadership skills. Ashgate Hospice is committed to supporting professional development through specialist education, clinical supervision, and opportunities to contribute to audit, quality improvement, and service development, with a clear development pathway towards Specialist Palliative Care Nurse.

Main duties of the job

palliative and end-of-life care to patients and those important to them across North Derbyshire. You will undertake holistic assessments, develop and evaluate personalised care plans, support symptom management and Advance Care Planning, and contribute to complex case management, clinical triage and prioritisation. You will manage your own caseload, work collaboratively with colleagues across health and social care, and provide support, advice and education to patients, families and professionals. You will also contribute to clinical governance, audit, quality improvement and the ongoing development of the Community Specialist Palliative Care Service. The role involves community visits, lone working and the use of telephone and digital systems to support patients and carers. Flexibility is required to participate in weekend and bank holiday working as part of the service rota.

Job responsibilities

This is a developmental specialist nursing role within Ashgate Hospices Community Specialist Palliative Care Team.

You will be supported to develop your clinical expertise in specialist palliative care through structured professional development, clinical supervision, reflective practice and participation in the teams competency framework.

Development opportunities include:

There is a clear development pathway within the role. Following successful completion of the development portfolio and demonstration of the required experience, knowledge and specialist competencies, there is an opportunity to progress to Palliative Care Specialist Nurse. Progression will be based on individual readiness and evidence of capability.

The role is based within our Community Specialist Palliative Care Service and involves working across North Derbyshire. The service currently operates five days a week and is expanding to six days, with the longer-term ambition of developing towards a seven-day specialist community nursing service.

The role requires flexibility, including weekend and bank holiday working, community visits and lone working. You will work closely with colleagues within Ashgate Hospice and across the wider health and social care system to provide coordinated, patient-centred care.
