At Sandwell and West Birmingham NHS Trust we are very proud of our specialist palliative care and end of life care co-ordination Hub. The CQC awarded us excellent for our service in 2017. The team consists of Specialist Palliative Consultants, Specialist Palliative Care Nurses for the Acute and Community, End of Life Care Facilitators, Urgent Response Team, Day Hospice and Macmillan Occupational Therapists. The service operates 24/7 providing care to patients in hospital and community in Sandwell and West Birmingham.

Main duties of the job

We currently have a vacancy available for a Palliative Care Nurse. You must be a registered nurse with the appropriate qualifications and experience. The applicant for this post will be responsible for coordinating a caseload of patients and staff on a shift, to ensure that all calls from palliative and end of life care patients are responded to in a timely manner.

The post includes rotation between the Palliative Care Hub and Urgent Response Team and offers development opportunities. You will need to have a good clinical knowledge around end of life care, including symptom control and community services. The shift pattern involves rotation between day and nights, 12 hour shifts.

  • Experience of working within a community team or acute ward.
  • Experience of caring for patients at the end of life / palliative care needs

  • Experience of forging excellent relationships across professional boundaries

  • Experience of working with professionals from health and other sectors including social care and the 3rd sector

Qualifications

Essential

  • 1st Level Registered General Nurse (adults)
  • Qualification in palliative / end of life care or relevant experience
  • Evidence of continued professional development
  • Holds a teaching /assessing qualification

  • Qualified Nurse Prescriber

  • Counselling
