Job summary

An opportunity has arisen for an enthusiastic and motivated Mental Health Nurse to join our Community based multidisciplinary team (MDT) of Physiotherapists, Nurses and Health Care Trainers. Pain management experience is desirable. Candidates to be able to demonstrate an interest in the speciality. They will work within the Community Pain Management Service to provide a person-centred approach to managing persistent pain. A qualification in independent non-medical prescribing is desirable but not essential. You will work autonomously within the service, managing a caseload of patients delivering individualised patient care. Your caseload will consist of patients with persistent pain and you will work with them to improve their quality of life by optimising their mental health. Strong communication skills and a compassionate approach to patient care are essential. The Service works closely with the Tier 3 pain management team and the Community MSK Service. You will participate in the delivery of group pain management sessions to provide education and advice face to face and on-line. Are you passionate about pain management? Main duties of the job

The role will include : Undertaking face to face and telephone assessments with patients with persistent pain- producing a jointly agreed Personalised Pain Management Plan. Using your previous Pain Management experience to support patients to achieve their Personal Pain Management goals, on a one to one and group basis. Delivering Pain Management group sessions. Working with other members of the clinical team (APP Physiotherapists, Pain Specialist Nurses, Physiotherapists, and Health Care Trainers) to support patients to live a better life with pain.
