We are seeking a Paediatric Diabetes Specialist Nurse to join our well-established, highly regarded multidisciplinary team. This is an exciting opportunity to become part of an award-winning service at the forefront of diabetes care for children and young people, including the use of advanced technologies such as closed loop insulin pumps and continuous glucose monitoring.

The team provides care across Calderdale and Huddersfield, Our service operates from the newly developed Rainbow Community Hub in Elland, offering a modern and supportive working environment.

This is a varied and rewarding role, involving regular visits to children and families in their own homes, as well as supporting care within school settings. Due to the community-based nature of the role, access to a car is essential.

Main duties of the job

To provide evidence based specialist care to children, young people and their families  with Type 1  Type 2 diabetes in Calderdale and Huddersfield

To manage your own caseload of patients and to make clinical decisions on a daily basis, involving altering insulin doses, offering telephone support to children, young people and their families, as well as supporting school staff and other professionals

To attend multidisciplinary clinics on a weekly basis,

To attend MDT meetings within the team as well as regional Nursing Network meetings

To attend multidisciplinary meetings supporting children that require safeguarding intervention

To commence pump starts and provide continued support and information to the children on pump therapy

To provide structured education to families and relevant professionals involved in the care of the children and young people

To be involved in compiling audit data

Detailed job description and main responsibilities

You will be responsible for managing your own caseload alongside the dietitian and patient advocate. You will also be responsible for supporting patients that are in our transition service alongside our adult colleagues and attending the evening transitional clinics.

You will attend regular follow up clinics, new patient clinics and nurse led clinics as well as visiting patients at diagnosis on the ward, and continuing their education in the community setting

QUALIFICATIONS / TRAINING

Essential

  • R.N. Child
  • Current NMC registration
  • Professional or clinical qualification relevant to the speciality (eg. Children’s Diabetes Modules) or evidence of working towards
  • Evidence of education to degree level
  • Willingness to undertake the Non-Medical Prescribing course

Desirable

  • Insulin pump therapy course

KNOWLEDGE, EXPERIENCE & EXPERTISE

Essential

  • Experience of caring for Children and Young People with Diabetes
  • Evidence of continual professional development
  • Safeguarding Children Level 3 training
  • Knowledge of Diabetes NICE guidance and diabetes best practice tariff.
  • Updated research and evidence-based practice
  • Demonstrate ability to assess, plan, implement and evaluate care

  • Organisational abilities
  • Appropriate IT skills

  • Experience of working in a community setting and managing a nursing caseload
