Job summary
Portobello Medical Centre is seeking a hard-working and proactive Practice Nurse to join our friendly and dedicated team.
Situated just off the Portobello Road, our practice delivers high-quality, patient-centred care to a diverse local population. In this role, you will deliver health promotion, chronic disease management and illness prevention. You will work alongside our GPs, HCA, clinical pharmacist and administrative staff to optimise patient outcomes.
Main duties of the job
Key Responsibilities
- Chronic Disease Management: Conduct regular reviews and create care plans for patients with long-term conditions (including Asthma, COPD, Diabetes, and Hypertension).
- Preventative Care: Administer routine childhood and adult immunisations, travel vaccinations, and seasonal flu clinics.
- Cervical Screening: Perform routine cervical cytology screening in line with national guidelines.
- General Nursing Care: Deliver standard treatment room services including wound care, ulcer dressings, and suture removal.
- Health Promotion: Provide lifestyle, dietary, and smoking cessation advice to promote long-term patient well-being.
- Infection Control: Adhere strictly to clinical governance, health and safety regulations, and infection control protocols.
- Record Keeping: Maintain accurate, simultaneous electronic patient records utilizing our primary care clinical software (SystmOne).
