An exciting opportunity has arisen to join the National Hospital for Neurology & Neurosurgery as a Motor Neurone Disease (MND) Care Centre Coordinator (Band 7).

This is a fixed term post for 12 months covering maternity leave, supported by a Motor Neurone Disease Association (MNDA) grant.

Working within a nationally recognised MND Care Centre, you will contribute to the coordinated, proactive and multidisciplinary care model recommended in NICE NG42. The role focuses on collaboration, working closely with the MND Clinical Nurse Specialist, Advanced Nurse Practitioner, Consultant Nurse, Neurologists and the wider MDT to ensure seamless, person centred support from diagnosis through to ongoing management.

You must have an understanding of the needs of patients with a life limiting illness; possess excellent communication skills for advising and supporting patients, relatives and other health professionals along with a willingness to learn about Motor Neurone Disease (MND)

We welcome applications from experienced Nurses or Therapists who demonstrate advanced communication skills, sound clinical reasoning and a commitment to teamwork and service improvement. You will join a supportive, forward thinking team dedicated to improving outcomes and experiences for people living with MND.

This is a full time, Monday to Friday role with no weekend or bank holiday working an opportunity to make a meaningful difference within a collaborative, specialist environment

Main duties of the job

The Band 7 MND Care Centre Coordinator will hold an appropriate professional qualification (RN or AHP) and work collaboratively within the MND team and wider multidisciplinary network to deliver coordinated, proactive and person centred care in line with NICE NG42 guidance. The postholder will: •    Practice autonomously while working in partnership with the MND Clinical Nurse Specialist, Advanced Nurse Practitioner, Consultant Nurse, Neurologists and community based MDTs. •    Coordinate care for people living with MND by attending borough specific MDT meetings, facilitating referrals and maintaining accurate records on the MND register. •    Contribute to the development and review of protocols, pathways and organisational guidelines, ensuring compliance with NICE NG42 and the National Service Framework for Long Term Conditions. •    Act as a clinical resource for colleagues and students, promoting evidence based practice and supporting education within the specialist area. •    Participate in audit, service evaluation and quality improvement initiatives to enhance patient experience and service delivery. •    Continue to develop specialist clinical knowledge and apply research and critical analysis to practice. •    Build strong clinical networks and maintain effective communication across acute, community and voluntary sectors. •    Demonstrate commitment to equality, diversity and inclusion, and uphold the Trust’s values of safety, kindness, teamwork and improving.

Essential

  • Substantial post-registration experience working at senior level in relevant field.
  • Experience coordinating complex care across acute, community or palliative settings
  • Experience working autonomously and making complex clinical decisions within scope of practice
  • Experience delivering person centred care for people with progressive or long term neurological conditions
  • Experience contributing to audit, service evaluation or quality improvement
  • Experience teaching or supporting learning for patients, carers or staff
  • Experience participating in clinical governance and responding to clinical incidents
  • Experience applying research or evidence to improve clinical practice
  • Experience supervising or mentoring junior staff
  • Experience contributing to service development, innovation or pathway design
  • Experience contributing to research, publications or presentations

Knowledge and Qualifications

  • Registered Nurse (Adult) OR AHP qualification
  • Honours Degree in Nursing, or AHP speciality or related subjects OR International equivalent
  • Evidence of advanced communication skills
  • Relevant post registration programme (neurology, palliative care, long term conditions or similar)
