We have recently been rated as outstanding by the CQC. This position is primarily based in the Bristol Haematology & Oncology Centre (BHOC) and the Bristol Royal Infirmary (BRI).

You will provide a ward based Medicines Management Pharmacy Service for cancer patients at BHOC supporting our inpatient, outpatient and day case patients.   You will perform medicines reconciliation on the ward, counsel patients and supply medication as appropriate. You will work within a multi-disciplinary team on the wards, including the specialist pharmacists and Macmillan Medicines Management Technicians for Cancer Services. Main duties of the job

To provide patient-focused pharmacy services to designated clinical areas (inpatient wards, outpatient services and day case units) in conjunction with clinical pharmacists and the dispensary team

Provide Medicines Optimisation Pharmacy Service for cancer patients at the Bristol Haematology and Oncology Centre, supporting inpatienst, outpatients and day case patients. The post-holder will perform medicines reconciliation on the ward & supply medication as appropriate, identifying and resolving medication related problems within a multidisciplinary team.

Provide a ward based dispensing and counselling service.

Provide safe and efficient pharmaceutical support to the dispensary.  This role includes the dispensing and accuracy checking of medicines and preparation of pharmaceutical products.

Responsible for all elements of the dispensing process to final check, for inpatient, outpatient, discharge, controlled drugs requests.
