<p><strong>Make a lasting impact on the care of older people across inpatient and community services.</strong></p><p>We are seeking an experienced and compassionate <strong>Medical Lead </strong>to provide clinical leadership and expert medical care across our inpatient community units in Swindon and North Wiltshire. This is an exciting opportunity to shape the future of integrated care services for older, frail patients with complex health needs, while supporting the development of a high-performing multidisciplinary team.</p><p>Reporting to the <strong>Medical Director, BSW</strong>, you will play a key leadership role within HCRG Care Group, driving excellence in patient care, supporting medical colleagues through job planning and appraisals, and contributing to the strategic development of our inpatient services.</p><p><strong>About the Role</strong></p><p>As Medical Lead, you will provide medical oversight and care for patients within our inpatient community units, focusing on older adults living with frailty, multimorbidity, and advanced clinical needs. Working closely with multidisciplinary teams, you will help deliver safe, effective, person-centred care that supports recovery, rehabilitation, and timely discharge.</p><p>You will be based predominantly across:</p><ul><li><strong>Swindon Intermediate Care Centre</strong> - 60
