We are seeking exceptional personnel to join us in delivering best practice personalised care and support interventions to cancer patients and their families as part of our Living with and Beyond Cancer Program here at Sandwell and West Birmingham Hospital NHS Trust. You will be joining a team of Cancer Care Navigators helping patients to manage the practical and emotional impact of cancer, who will drive forward the role to ensure it is fully embedded and integrated into our cancer pathways. You will also work with the wider LWBC team in delivery of Health and Wellbeing events.
We are seeking people who can exercise their initiative, use independent judgement and make decisions within a variety of situations.
Main duties of the job
Working closely with Cancer Services and our Cancer Nurse Specialist Team’s (CNS) you will be a source of contact for cancer patients and their families, helping them to navigate through their cancer pathway, from diagnosis and into follow-up; providing reassurance, telephone patient triaging and support, along-side patient-related information giving and signposting. You will have support and guidance from our Cancer Care Navigators and the LWBC facilitator.
You will be required to assist the CNS teams in the smooth running of their living with and beyond cancer services which will include undertaking administrative duties relating to nurse-led clinics, MDT Outcomes and the tracking of patients’ on remote surveillance. You will also be responsible for delivering Holistic Needs Assessments and Care Planning telephone clinics supporting patients in the resolution of non-complex needs and concerns. As well as managing patients who are on patient stratified follow up through remote monitoring
Detailed job description and main responsibilities
The Post holder needs to have enthusiasm, commitment and a drive to work with cancer patients and carers. The ideal person will have expertise skills and knowledge of cancer. The ability to learn and develop there knowledge and abilities to carry out Holistic Needs Assessments on patients through the journey and to develop care plans. Sign posting to service that will support the patient to self manage is the key to success.
You will have excellent communication skills and be confident in dealing with patients both on the phone and face to face; whilst being aware of the limitations and boundaries of your expertise level and role. These key skills need to come hand in hand with demonstrable evidence of competent IT and office administration skills, along with experience of information patients systems to sustain accurate and timely electronic patient records.

Qualifications
Essential
- GCSE English language and Mathematics (Grades A-C) or equivalent
- NVQ 3 plus additional training
- Recognised ECDL or equivalent or extensive experience using Microsoft office package
Desirable
- Communication skills training i.e. Thyme and Sage
