
NHFT is contracted to provide integrated health care services to HMP Littlehey. Currently we are looking to recruit an innovative Band 7 long term condition lead to our existing team. The post holder will deliver the highest standard of care required to meet the diverse and complex healthcare needs of an adult male population. They will be passionate about all aspects of Nursing, have exceptional clinical skills and hold an NMP qualification. Demonstrating consistent exemplary standards of care and professionalism.
HMP Littlehey is a large Category C prison located in the picturesque village of Perry near to Grafham Water near Huntingdon in Cambridgeshire with free parking, staff restaurant and an onsite gymnasium.
Healthcare services are delivered from two modern well equipped healthcare centers by a multi-disciplinary team who have, in the past, won the prestigious WHO European Best Practice in Prisons Award. The Primary care and Long-Term Conditions (LTC) clinical lead working in a prison is responsible for delivering high-quality, evidence-based care to people in custody with chronic health conditions, ensuring care is equivalent to that provided in the community.
The role involves overseeing nursing staff while assessing, monitoring, and managing long-term conditions through regular clinics and reviews. You will also be developing individualized care plans and supporting safe and effective medication management. You will work autonomously and as part of a multidisciplinary team, promoting self-management and health education while balancing clinical priorities with the constraints of a secure environment. The post holder will also play a key role in safeguarding, risk management, accurate clinical documentation, and ensuring continuity of care during transfers and on release.
