Could you work in  a busy and dynamic team, supporting individuals over the age of 65 with both organic and functional illness?

The Derby City  Older Adult Community Mental Health Team (CMHT) are looking for an enthusiastic Lead Nurse who has a specific interest and passion in caring for older adults with Organic and Functional Illness.

Ability to travel is essential.

Main duties of the job

The role of Lead Nurse includes supporting and managing a complex caseload, initial assessments, assessing risk, developing collaborative care plans, undertaking the role for duty worker, administering depots, attending CST meetings and working closely with our partners in health and social care.

As a Lead Nurse there is a requirement to support and develop Student Nurses and to supervise junior members of the team.

Duty working is also required as part of the role.

The successful candidates will receive regular clinical and managerial supervision, with access to professional development.

In the Older Peoples Division, development of knowledge and skills is actively encouraged, with training opportunities supported wherever possible.

  • Extensive knowledge of the principles of Safety planning
  • Experience of post qualifying working with older adult patients who have an organic and/or functional ill
  • Knowledge of patient centred care planning

Desirable

  • Post qualification community experience

Attainment/ Qualifications

Essential

  • Registered RMN
  • Degree/ Diploma in Nursing
  • Mentorship qualification or equivalent
