The role of the IBD Biologics and Patient Pathway Co-ordinator is integral to the Gastroenterology Department for tracking, developing and monitoring the pathways of care for IBD patients.

Working as a member of Inflammatory Bowel Disease (IBD) Specialist Team the post holder will provide a combination of support and administration duties in the delivery of services for patients with Inflammatory Bowel Disease making a direct and positive contribution to the work of the team. The post holder must exercise initiative and judgement when dealing with patient enquires, requests, referrals and results, and be able to prioritise importance accordingly.

The post holder will have responsibility for the organisation and delivery of the weekly multi-disciplinary team meetings and follow up decisions taken. Ensuring all communication fully completed and shared where required.

Main duties of the job

  • MDT meetings. Act as a point of contact for the IBD Specialist Team for primary and secondary care healthcare professionals, Collaborate with multidisciplinary teams and external agencies to ensure efficient and effective delivery of services across primary and secondary care. Liaise with clinical teams to coordinate patient care, appointments, day-case procedures, investigations, treatments and prescriptions, ensuring a seamless patient experience. Coordinate patient prescriptions, including homecare and healthcare services. Assist the nurse specialist team with monitoring blood results for patients receiving IBD therapies, recognising abnormalities and escalating concerns promptly to support safe ongoing treatment. Assist in the development and maintenance of databases and data collection for audit, education, surveillance and service evaluation purposes. Coordinate the collection and submission of local and national audit data as required. Organise meetings for the team Coordinate patient satisfaction questionnaires, Assist in the development and maintenance of the IBD intranet site and other service resources Prioritise workload effectively, working independently with minimal supervision

Detailed job description and main responsibilities

  • Main Duties and Responsibilities
  • Booking of IBD referral pharmacy clinics and coordinating the alternate-week Pharmacy/IBD MDT meetings.
  • Deal with all enquiries in a responsive and professional manner, liaising with staff across all disciplines within and outside the Trust to maintain effective communication.
  • Listen to advice line telephone and email messages, enter them onto the Outlook taskbar, deal with non-clinical queries and liaise with clinicians where appropriate.
  • Complete patient care records for all non-clinical queries managed.
  • Use judgement and initiative to prioritise the urgency of patient messages, enquiries, requests and results.
  • Act as a point of contact for the IBD Specialist Team for primary and secondary care healthcare professionals, communicating information relating to patient treatment, prescriptions and care requirements.
  • Be responsible for incoming correspondence, assessing priority, responding appropriately and following up actions arising from correspondence, meetings and delegated tasks, using negotiation and persuasion skills where required.
  • Work with clinical, management and divisional leads to identify issues within patient pathways and escalate concerns appropriately.
  • Collaborate with multidisciplinary teams and external agencies to ensure efficient and effective delivery of services across primary and secondary care.

  • Develop and maintain a good working knowledge of inflammatory bowel disease, relevant anatomy, treatments, medications and medical terminology/coding.
  • Assist the nurse specialist team with monitoring blood results for patients receiving IBD therapies, recognising abnormalities and escalating concerns promptly to support safe ongoing treatment.
  • Assist in the development and maintenance of databases and data collection for audit, education, surveillance and service evaluation purposes.
  • Coordinate the collection and submission of local and national audit data as required.
  • Organise meetings for the team, including booking venues, preparing agendas, training materials and education resources.
  • Coordinate patient satisfaction questionnaires, collate responses and support analysis to inform service improvements.
  • Assist in the development and maintenance of the IBD intranet site and other service resources.
  • Maintain accurate electronic and paper patient records, including filing documentation on Evolve where appropriate.
  • Prioritise workload effectively, working independently with minimal supervision while recognising personal limitations and seeking support when required.
  • Communicate confidently, professionally and sensitively with patients, carers and colleagues.
  • Participate in service improvement initiatives and support the implementation of service developments within the Division and Trust.

Communication and Working Relationships

  • To work closely with the Consultant, Nursing and Senior Medical Secretariat teams to ensure tasks are completed.
  • To communicate complex, sensitive, contentious information at times.
  • To liaise with patients to arrange outpatient appointments.
