We are delighted to offer an exciting opportunity to join the Coventry and Rugby Heart Failure Nursing Team as a Band 6 Heart Failure Specialist Nurse on 9 month secondment or fixed-term. The post is 37.5 hours but other hours will be considered

The role will be primarily based at University Hospital Coventry and Warwickshire (UHCW), with opportunities for cross-site working at Hospital of St Cross, Coventry Community Services, as well as home visits and working from home.

The role will involve reviewing and clinically assessing patients with heart failure, providing specialist advice and recommending evidence-based therapies, including appropriate medications.

Applicants will need to hold a full driving licence and have access to a car to undertake the requirements of the role.

We are looking for applicants who have:

• Current Registered Nurse status and relevant post-registration experience.
• Essential knowledge and experience in the management of patients with heart failure.
• Strong clinical assessment and decision-making skills.
• The ability to work autonomously as well as effectively within a multidisciplinary team.
• Excellent communication and interpersonal skills.
• Good IT skills.
• A full driving licence and access to a car.
• Venepuncture and clinical assessment skills are desirable

Main duties of the job

We are looking for a motivated and compassionate Registered Nurse with post-registration experience and a passion for delivering high-quality care to patients living with heart failure.

The successful candidate will be involved in the assessment, management and ongoing care of patients with heart failure across a variety of settings, including:

• Inpatient wards
• Virtual clinics
• Virtual ward
• Patients’ homes
• Telephone consultations

The role will involve reviewing and clinically assessing patients with heart failure, providing specialist advice and recommending evidence-based therapies, including appropriate medications and cardiac devices.

Successful applicants will be required to provide an immunisation and vaccination report, from either their current UK occupational health provider or their UK GP.

Detailed job description and main responsibilities

Job Summary

To support patients and carers with stable and unstable heart failure. To promote quality of life, contributing to the prevention of avoidable hospital admissions and readmissions. To contribute to developing robust guidelines and protocols and providing clinical advice and education ensuring seamless pathways of care for patients with heart failure. To support advanced care planning strategies for end stage heart failure patients.

Main duties

• To provide and evaluate nursing care for a designated caseload of individuals with heart failure.
• Provide clinical assessment of patients using clinical knowledge and skills as per heart failure management guidelines
• Provide ongoing clinical support and monitoring to optimise heart failure medications in conjunction with consultant cardiologist or GP
• Monitor clinical variations including blood biochemistry or assist the patient in monitoring clinical variations and take appropriate action to optimise treatment of the condition and prevent an avoidable admission to hospital
• Arrange referrals for further investigations in consultation with consultant cardiologist or GP if necessary
• To support patients in the most appropriate location i.e. ward, home, outpatient clinic, nurse led clinic and hospital at home.
• To ensure a consistent approach to patient care, including access to and provision of appropriate patient and carer information and education programmes.
• Ensure continuity of care by making sure that patients are referred to appropriate agencies for additional support or treatment at the most appropriate time such as community matrons, palliative care, social services and voluntary support groups
• Where possible encourage patients and carers to self manage and make best use of non-specialist services e.g. District Nurses, Social Services
• To act as a resource, providing specialist training and support for other health care professionals within primary and secondary care
• Identify needs associated with managing end of life outside of hospital and in accordance with patients/carers wishes and co-ordinate services in a timely manner
