Lewisham and Greenwich NHS trust has an exciting opportunity for Healthcare Support Workers to join the Adult Community Nursing Service delivering services to patients in their own homes in one of our 4 local neighbourhoods.
As no two days are the same in the community, you will be able to work with a high degree of autonomy and professional accountability where the work you do makes a real difference to your patients and those important to them, in line with our trust values.
A full UK driving licence and use of car is essential for this role.

- A valid Care Certificate issued by an NHS provider organisation (Care Certificates issued by private organisations are not accepted).
- An NVQ Level 3 in Health and Social Care qualification obtained from a UK college.

Detailed job description and main responsibilities
The job role includes monitoring patients’ conditions by taking vital signs and escalates changes in patients’ conditions to the district nurse or senior nurse on duty
Taking blood for testing (Phlebotomy) and administration of insulin following appropriate training and consolidation
Carrying out simple wound dressings and other relevant nursing tasks as assigned by the senior nurse.
Promotes patient independence through implementing rehabilitation programmes as directed by the multi-professional team
Undertakes health promotion activities with patients and their families as directed by the District Nurse or members of the multi-disciplinary team
Responsible for ensuring up to date stock levels of materials and equipment used by the team
Maintains accurate documentation immediately after nursing intervention in line with Trust policies, guidelines and procedural documents
To use mobile working devices to record and document relevant information
Undertake routine audits as required
The two shift patterns for the role: 08.00 – 16.00 and 12.00 – 20.00 including weekends and bank holidays.
The post holder must have a full current UK driving licence and daily access to a car, as this is essential for the role

Knowledge
Essential
- • knowledge of Infection control, skin integrity, the value of nutrition etc.
- • knowledgeable about activities of daily living and the effect illness has on people who are debilitated and dependant on others for their care
- • knowledge of Trust policies, procedures and guidelines and relevant legislation; confidentiality, information governance etc
Desirable
- Basic knowledge of issues relating to health and wellbeing and patient care.
Experience
Essential
- Experience of working in formal or informal environments such as peoples’ homes, community, statutory, voluntary or paid employment
- Customer service experience
Skills and Attributes
Essential
- • Ability to work under pressure, responding with flexibility to the changing demands of the workload
- • Ability to follow a clear instruction and feed back to senior staff.
- • Ability to work as part of a team, as well as on own initiative.
Desirable
- Ability to undertake: Blood glucose monitoring; taking vital signs; do basic wound care; pressure area care etc
Qualifications and Training
Essential
- Educated up to GCSE with English language
Desirable
- NVQ Level 3 in Health and Social Care, or willingness to undertake training.
- Evidence of continuous learning in any area and willingness to learn practical nursing skills on the job
