Main duties of the job
The Health Navigator / Receptionist role at Chelston Hall Surgery is an opportunity to become part of a forward-thinking and dynamic team committed to integrated multi-disciplinary working. With a patient list size of around 18,500 across two sites, the focus is on delivering excellent primary care that enhances access, outcomes, and patient experiences. Main Responsibilities:
As a valued member of the Operose Health team, your key duties will include:
- The post holder will work to support timely and personalised care for patients, helping to help them ensure they have the right support at each stage of their journey. They will have a positive, can-do attitude and be responsible for patient registrations and deductions as well as providing a range of administrative support for the Patient Communications team.
- In this role you will be dealing closely with different people on a daily basis, both face to face, over the telephone and via email. You will be supporting and co-ordinating care for a range of patients including those with long term health conditions, multiple LTCs, frailty or high risk. We are looking for someone with a confident and patient focused approach to their work, who is able to problem solve and adapt to change.

- Reception or customer care experience is essential as well as an excellent communicator both spoken and written.
- Must have basic PC skills such as Word, Excel and email.
- Able to work within processes, procedures and maintain confidently and data security.
- Previous experience of working in the NHS is welcome but not essential.

Essential
