JOB DESCRIPTION
JOB SUMMARY
Introduction
The Learning Disability Health Facilitation Nursing Service Band 6 role has been developed to promote equitable access to healthcare for people with learning disabilities by working collaboratively with service users, carers, primary care, secondary care, and community services. The post holder will provide specialist advice, support, education, and health facilitation to reduce health inequalities and improve health outcomes.
The post holder will be responsible for providing training, guidance and support in relation to health promotion, prevention, facilitation and screening.
The post holder will focus and lead under the direction of the Band 7 on the following key areas, within the organisation’s governance framework:

Act as a specialist resource for individuals with learning disabilities, families, carers, and healthcare professionals.
Undertake comprehensive health assessments and identify unmet health needs.
To explore, develop and recommend ways of identifying and improving the health needs of individuals with Learning Disabilities (and their Carers).
Support the development and implementation of person-centred health action plans.
Support access to mainstream health services and ensure reasonable adjustments are made.
Contribute to reducing health inequalities and improving health outcomes.
Support the delivery and monitoring of annual health checks.
Promote health screening programmes and preventative healthcare initiatives.
Promote health, intervention and prevention through participation in the health promotion steering group.
Manage a defined caseload and prioritise workload effectively.
To maintain effective links with the Health Development Department.
To adhere to the NMC Code ‘Standards of Conduct, Performance and Ethics for Nurses and Midwives’.
To ensure a high quality, evidence- based approach in the delivery of health facilitation and screening.
To function as an independent practitioner while communicating and advising primary health care teams, learning disability teams, GP’s and other professionals.
To participate as an integral member of a multi-disciplinary team in discussion meetings, multi-disciplinary case discussion, reviews and meetings as appropriate.
To maintain high quality accurate manual and computer records of all aspects of clients’ care within agreed time scales.
To liaise with other professionals and agencies as appropriate.
Liaise effectively with GPs, hospitals, community teams, social care, and voluntary agencies.
Advocate for people with learning disabilities to ensure their voices are heard.
Deliver accessible health information using appropriate communication methods.
Participate within the Regional Health Facilitation meetings, actively contributing to service improvements and better outcomes.
Maintain accurate and timely clinical records.
Education and Training
Identify and deliver training and awareness sessions to patients, families/carers and healthcare staff.
Promote understanding of reasonable adjustments and legal responsibilities.
To assist in regular clinical audit of Health Screening and Facilitation work including user/carer satisfaction survey.
To provide teaching, clinical presentation and representation at meetings to multi-disciplinary teams and services.
Participate in service development initiatives.
To keep up to date with learning disability and primary health strategies and actively seek to advance his/her practice and ensure that high quality evidence-based care is delivered.
Contribute to nurse education to both pre-registration and post-registration levels and participate in the provision of relevant programmes of experience by teaching and supervising during placement.

Ensure that equality and diversity issues are respected and that patients / clients and their carers’ are empowered to be equals in their journey of care.
Contribute to the development of strategies for communication between staff, patients / clients, relatives and their carers’, showing awareness of barriers to understanding.
Communicate effectively between all members of the interdisciplinary team, and across care settings in a manner consistent with the post.

Identify opportunities to the Band 7 for meaningful involvement of patients / clients and carers in relation to the development of care and services.
Assist the Band 7 to ensure effective systems are in place to gain patient / client and carers’ feedback on their experience of care.
Ensure compliments and complaints are managed in line with HSC Trust policy including the dissemination of shared learning.

Support implementation of local and national policies relating to learning disability healthcare and participate in regional meetings.
Assist with supervision and mentorship of junior staff and students where appropriate.
Establish and maintain relationships based on mutual respect communicating on a regular basis with the patient / client, relatives and carers’ in the provision of care and services.
Maintain active status on NMC register in accordance with NMC requirements
Act always in accordance with NMC Code of Conduct and guiding documents and adhere to Trust Policy and Procedures
Knowledge of Safeguarding, Mental Capacity and Restrictive Practices, including Deprivation of Liberty Safeguards
Adhere to the General Data Protection Regulation (EU) 2016/679 and Data Protection Act (2018).
Maintain effective records management ensuring all documentation written or electronic is managed as per HSC Trust and NMC (2009) guidelines.
The post holder will develop personal knowledge and skills to level 2 – 4 of the SBNI ‘Child Safeguarding Learning and Development Strategy and Framework’ (2020 – 2023).

Environment
To maintain a safe and clean environment for staff, patients / clients and visitors by ensuring compliance with legislation, policies and protocols including health and safety, healthcare associated infection, risk management and critical incident reporting.
Ensure staff awareness of environmental issues and take appropriate action as per HSC Trust policy.
Contribute to the analysis, assessment and management of actual and potential risks to health and well-being.
Ensure safe and effective use of equipment as per HSC Trust policy.
Ensure near misses, incidents, accidents and faulty devices are recorded, reported, investigated and learning disseminated as per HSC Trust policy.

Staff Management and Develop team performance

Support with the analysis of areas of need, under the guidance of the Band 7, working collaboratively with identified gaps in service provision, supporting the co –design and development of new initiatives and management of services that takes account of the needs of patients / clients, their families and carers.
Organise own work and manage own practice, identifying and maintaining supportive networks
Promote and lead on effective preceptorship for newly registered nursing colleagues.
Role model to ensure supervision is embedded for pre and post registration students, registered nurse, nursing assistants and senior nursing assistants as appropriate.

Working Hours
While it is anticipated hours of work will primarily be Monday to Friday, 9am–5pm, the successful applicant will be required to be flexible in their working hours to meet the needs of patients as required.
