Job overview

We are looking for an enthusiastic and motivated Health Care Assistant to provide quality care within packages of care provided by the Continuing Health Care Team (CCHC).

The CCHC is part of the Salford Children's Community Nursing Service. We provide commissioned packages of care for young people with complex and/or palliative care needs. Care is provided within a range of settings including the child’s home, education and community settings.

We require the candidate to be caring and compassionate with excellent communication and time management skills and be able to respond to unexpected and challenging situations appropriately and professionally.

A full individualised structured induction programme will be offered and training on a range of clinical skills including monitoring the young person’s vital signs, respiratory support including care of a tracheostomy; ventilation and secretion management; nutritional support and medicine management; as well as supporting the family with day-to-day care need.

Regular coordinated support will be offered with a range of supervision sessions; this will as well as a chance for the candidate to allow to contribute to our services ethos of placing children, young people, and families first; whilst enhancing the Northern Alliance Care trust values

The role includes different shift patterns over a 24-hour period which is predominantly night cover but will also include days shifts.

Main duties of the job

  • To work unsupervised caring for the young people in a variety of settings (home, school, community, hospital).
  • Work on a shift pattern that includes days and nights
  • Carry out all agreed therapeutic programmes and interventions effectively. This includes monitoring and recording the young person’s vital signs, respiratory support including care of a tracheostomy; ventilation and secretion management; nutritional support including enteral feeding and medication management; as well as supporting the family with activities of daily living.
  • To communicate with the young people and their families in an appropriate manner
  • To plan and provide play and stimulation when appropriate according to the young person’s needs and abilities and when appropriate following their individual care plans by other therapists and professionals.
  • Clear and relevant documentation in patients records both written and electric where appropriate.
  • To take an active part in all learning opportunities and maintaining your own personal development.
  • To support and assist in the mentoring of new team members during their induction.
  • Follow up to date infection control procedures whilst caring for the child in any appropriate setting including the wearing of appropriate PPE for example mask/ visors/gloves and aprons.

Qualifications

Essential

  • NVQ Level 3 in Healthcare or a Level 3 Senior Healthcare Support Worker Apprenticeship - or equivalent experience
  • Level 1 Literacy and Numeracy (GCSE English and Maths Grade D-G)
  • Care Certificate

Skills & Abilities

Essential

  • Good communication (written and verbal skills)
  • Basic IT skills
  • Good organisational skills
  • Ability to travel to different geographical locations across the Trust footprint in line with service needs

Desirable

  • Full UK driving license with willingness to use own car for work purposes

Knowledge, Training & Experience

Essential

  • Experience of working as a Care Support Worker or equivalent in a Health or Social care setting
  • Experience of working within a team

Desirable

  • Demonstrates an interest or previous experience in supporting other health care staff and learners
  • Commitment to undertaking continuous professional development (CPD)
