We have an exciting opportunity for a Developmental Clinical Nurse Specialist (CNS) specialising in Dementia and Delirium (DaD) to join St Georges, Epsom and St. Helier (GESH) NHS Trust, to work across ESTH site.

As part of our commitment to improving care for patients with cognitive impairment, we are seeking an experienced and motivated nurse with a passion for older people’s nursing, and a specific interest in dementia and delirium care, to join our Fundamentals of Care Team. This is a developmental role, offering structured support, supervision and learning opportunities to enable progression towards a specialist DaD CNS position.

Main duties of the job

Ensure that patients always receive a high level of fundamentals of care. Assist the CNS to contribute to the monitoring of this through audit or other KPIs/ or outcome measures.

Ensure effective patient centred care is provided to patients who have or are at risk of complications related to your speciality of Dementia and Delirium

Work towards developing Advanced Nursing knowledge and skills to enhance the holistic management, experience and pathways for patients with Dementia and Delirium.  Take personal responsibility to ensure your own personal and professional development, which may include any competency frameworks in your speciality of Dementia and Delirium.

Support all aspects of the Dementia and Delirium service across the Trust with the CNS and other team members.

Provide information and advice to healthcare professionals about the DaD service using your clinical judgement. Act as an information resource on Dementia and Delirium related issues in line with your role.

Assist with the provision and delivery of education and training (both formal and informal) about the DaD service, to all levels of staff and disciplines.

To highlight or escalate issues around effectiveness and efficiency. To manage any equipment required or specific to the Dementia and Delirium service

To support and assist with audits on Dementia and Delirium care in collaboration with the lead CNS to develop the DaD service where appropriate

In this role, you will work closely with the lead DaD CNS and colleagues, within the wider Fundamentals of Care Team, to help shape and deliver high-quality, evidence-based dementia and delirium care; ensuring patients and service users receive compassionate, person-centred care, aligned with national guidelines and best practice.

You will support the assessment, prevention and management of delirium, and promote dementia-friendly approaches across the acute setting.

You will develop specialist skills and knowledge through a clear development pathway, with the opportunity to grow in confidence and autonomy as competencies are achieved, working towards a specialist nurse role.

• The ability to work effectively within a small, supportive team and as part of the wider multi-disciplinary team
