The Frailty SDEC Clinical Nurse Specialist plays a key role within the Acute Assessment and Frailty Team (AAFT), delivering high quality, patient centred care to older people presenting as emergencies to Milton Keynes University Hospital. The post holder will work autonomously and as part of a multidisciplinary team to provide comprehensive geriatric assessment, support timely clinical decision making, prevent unnecessary admissions, reduce length of stay, and enable safe and effective discharge across the emergency care pathway, including ED and Same Day Emergency Care (SDEC). We are looking for an experienced, motivated nurse with strong clinical assessment skills, a sound understanding of frailty syndromes, and the ability to work confidently in a fast paced acute environment.   'We care We communicate We collaborate We contribute'

The ideal candidate will demonstrate excellent communication skills, advanced clinical reasoning, and the ability to collaborate effectively with medical, therapy, and community teams. Experience in frailty, acute medicine, emergency care, or complex discharge planning is essential, alongside a commitment to compassionate, dignified care for older people.

This role offers significant opportunities for professional growth, including development of advanced frailty expertise, leadership within the frailty pathway, involvement in service development, quality improvement, audit and research projects, and delivery of education and training across the Trust. The post holder will be supported with ongoing learning, appraisal, and career progression, with scope to develop advanced clinical, leadership, and service improvement skills within a growing and innovative Frailty SDEC service. Please note that this post is subject to an initial probationary period of six months, extendable up to a period of 12 months. A DBS check may be required for new employees.
