Kingston and Richmond NHS Foundation Trust is looking for an exceptional Frailty Nurse Consultant to lead the development of innovative frailty services and shape the future of urgent care for older adults.

This is an exciting opportunity for an experienced senior nurse with advanced clinical expertise, strong leadership skills and a passion for improving outcomes for people living with frailty. Working across our Frailty Same Day Emergency Care (SDEC) service, Emergency Department and Care of the Elderly pathways, you will play a pivotal role in delivering high-quality, person-centred care whilst driving service improvement across the organisation.

We're looking for someone who is:

  • A registered nurse with significant senior clinical experience in frailty, elderly care or acute medicine.
  • Educated to Master's level (or equivalent) with advanced clinical assessment skills.
  • A qualified Non-Medical Prescriber.
  • An experienced clinical leader who inspires and develops others.
  • Passionate about improving services through innovation, quality improvement and collaboration.
  • An excellent communicator who can build strong relationships across professional and organisational boundaries.
  • Committed to delivering compassionate, personalised care for older people.
  • Leadership qualification and experience of research, audit and service redesign would be advantageous.

Main duties of the job

As our Frailty Nurse Consultant you will combine expert clinical practice with strategic leadership, education and service development.

Working alongside consultants, advanced practitioners, therapists and the wider multidisciplinary team, you will:

  • Provide expert clinical leadership for patients living with frailty.
  • Deliver advanced clinical assessment, diagnosis and management, including independent prescribing.
  • Lead the development of frailty pathways across acute and ambulatory care.
  • Champion evidence-based practice and continuous quality improvement.
  • Support the education, supervision and development of nursing and multidisciplinary colleagues.
  • Lead service improvement projects, audit and research to improve patient outcomes.
  • Influence Trust-wide strategy for frailty services and integrated working across organisational boundaries.

This is a highly visible leadership role where you will have the opportunity to influence practice across the Trust and work with system partners to deliver the best possible care for older people.

  • Master’s degree or equivalent at master’s level.
