Are you looking for an exciting change of role in an established but developing service? We are looking to recruit  two dynamic and innovative Band 5 Flow Coordinators to join the Integrated Community Response Service- a multidisciplinary team made up of GPs, an Advanced Clinical Practitioner, occupational therapists, a social worker, physiotherapists, nurses, a community matron, and support workers.  The main aim of the service is to manage complex clinical cases at home, prevent unnecessary hospital admission and support early discharge of patients following an acute admission.

The team provides immediate assessment, intervention and management to a broad range of patients with complex needs. All members of the team work very closely together and have a good relationship with other community teams and clinicians such as district nurses, GPs, London Ambulance Service and mental health services.

Main duties of the job

We are looking for two enthusiastic and motivated candidates with experience of managing complex discharge pathways and experience of working with multiple providers to provide co-ordination and leadership in line with agreed service objectives.  The successful candidates will be required to contribute to the service staff roster from 8am to 8pm; seven days per week on a shift basis. The shift patterns available are either 12-hour shifts  (8am to 8pm) or a combination of early and late shifts (early = 8am to 6pm and lates = 10am to 8pm)

The postholders will work jointly with the Hounslow Integrated Discharge Hub, hospital teams and the London Borough of Hounslow to ensure patients are effectively supported through these pathways.

As the service is community based, you will ideally need to be a car driver or be able to access the community via bike or public transport.

  • Experience of providing performance data
  • Experience of working within services that are going through transformation projects
  • Experience of working within a team that incorporates multiple providers including Adult Social Care
  • Experience of working in community physical health services
  • Working within a nursing, physiotherapy and occupational therapy team within an inpatient or community setting
  • Experience of participating in QSIR/quality improvement projects
  • Experience of supervising, training and coaching staff

  • Working in or with discharge to assess and / or hospital at home pathways
  • Working in or with urgent community response pathway
  • Working in or with Single Point of Access
  • Experience of triaging

Knowledge and Skills

Essential

  • Understanding of community and hospital discharge pathways
  • Understanding of health and social care language and terms
  • Understanding of the Intermediate Care Framework for rehabilitation, reablement and recovery post hospital discharge, urgent community response and virtual wards.
  • Knowledge of Continuing Healthcare and community/social care processes
  • Knowledge of frailty models of care and comprehensive geriatric assessments and how to implement standards within community and inpatient settings
  • Ability to collect, process and convey complex clinical information to support referral triage and patient care planning
  • Ability to use a range of electronic patient records, Microsoft applications and other digital tools
  • Ability to communicate with vulnerable and unwell patients and concerned carers to support decisions regarding their care and support
  • Ability to develop and maintain excellent working partnerships with acute and community health staff, local authority staff, external agencies and service providers to provide the best support to patients
  • Ability to recognise the need for help and support from other professionals.
  • Understanding of the Mental Capacity Act 2005, in the context of consent, best interest decisions and Lasting Power of Attorney

Qualifications and Training

  • NVQ level 4 or above
  • Evidence of relevant CPD
