Through collaborative working with clinical areas daily, you will be contributing towards the assessment and review of patients requiring a higher enhanced level of care. The Enhanced Care Team provide increased visibility to maintain patient safety and promote a positive patient experience through the supported delivery of individualised care and therapeutic interventions.

The role involves the active organisation and delivery of training Trust wide in line with national guidance in the Dementia Strategy, the Butterfly scheme and John’s Campaign The role will play an active part, in partnership with others, to influence change and innovation in caring for people with dementia /and or delirium.
There is close working with the Dementia Lead matron to ensure ongoing development of skills and knowledge through relevant training, conference, and collaborative meetings. Working alongside relevant partners in the voluntary sector and local community are also encompassed within the service.
Main duties of the job
- To support the Enhanced Care Support Worker and provide professional leadership within the team.
- To support with managing the A level student enrichment programme, engaging with the enrichment leads in local colleges, promoting and supporting spread across the localities with support from the team coordinator.
- To undertake a holistic clinical assessment following referral of patients requiring the advanced level of 1-1 supervision requiring meeting their individualised care needs alongside the Clinical Lead for the service.
- To support with daily allocation of an ECSW to work in partnership with the ward team to provide enhanced care and intervention for the patient, supporting individualised care and therapeutic interventions in partnership with the patient’s family/carers.
- To demonstrate deliver of up-to-date practices, policies, and protocols within the clinical environment and in line with best practice within the national Dementia strategies.
- To support the Clinical lead with Quality Improvement projects across the trust, taking a lead on QI projects when required.
- To support the work undertaken with the Dementia Lead matron to ensure ongoing development of skills and knowledge through relevant training, conference, and collaborative meetings.

- MAIN TASKS REQUIRED OF THE POST

- 3. DUTIES AND RESPONSIBILITIES OF THE POST
Clinical Duties
- To support the care provision for patients on the wards who require enhanced levels of 1-1 care by the allocation of the enhanced care support workers.
- To take an active role in the risk assessment and planning of patient care in collaboration with the ward teams and supporting implementation of strategies to minimise risk.
- To ensure that accurate records are maintained reflecting changes in the patient’s treatment and interventions relevant to the care delivered by the service, in line with the Enhanced Care Policy.
- Communicate and collaborate across the integrated teams to facilitate timely patient/carer interventions and support and the effective transfer of care where this is deemed appropriate.
- Ensure that the care being provided has appropriate safeguards to protect the patient at risk from abuse and ensuring that the process offers the patient choice and support to make decisions about their safety and well-being. This will include providing advice around Mental Capacity assessments and DOLS.
- Act as patient advocate, supporting and promoting their involvement in decision making.
- To support with the provision of bespoke “on-the job” coaching training to all ward teams with regards to individualised and therapeutic interventions for patients who require enhanced care, role modelling to staff as part of their development.
- To establish and maintain effective communication, both verbal and written and relationships with multi-disciplinary team, carers, and other care settings as appropriate.
- To provide relevant advice and information to professional colleagues, patients, carers, and the public.
- To promote and provide relevant and clear information, education and advice for patients, carers, and families according to their needs
- To assist in the recruitment and selection of junior staff.
- Act as an innovative and enthusiastic role model, providing leadership, guidance, and advice to staff on operational & professional issues
- Attend and participate in meetings as required including providing feedback and disseminating information as appropriate.
Professional Duties
- Act as a role model for quality care, promoting the need for effective evidence-based interventions.
- Act as a patient advocate at all points in the patient’s pathway.
- To work within the NMC/HCPC Code of Conduct at all times.
- Adhere to national, local, and professional standards, guidance, and directives relevant to their sphere of practice
- To provide mentorship and preceptorship as required.
- To take responsibility for own continuing professional development and
- performance, including identifying own developmental needs and maintain own portfolio in accordance with registration requirements.
- To support the development of other team members
Managerial Duties
- Deputising for the Clinical Lead for Enhanced Care and support Team.
- To recognise, prevent where possible and manage areas of conflict, reporting to the clinical lead for the service if unable to resolve.
- Work in partnership with ward sisters/charge nurses, college enrichment leads and community teams to ensure the delivery of quality care.
- To be proactive in the development of the enhanced care and support team, supporting the clinical lead in service development and quality improvement, through the contribution of new ideas to improve patient care and experience.
- Support a culture in which individuals feel able to report incidents and that learning takes place from all incidents and complaints.
- To build relationships with MDT members across the trust.

- Participate in regular supervision.
- Attend all mandatory training.
- Participate annually identifying, developing and agreeing your own development plan with your Line Manager using the Trust Appraisal.
