An exciting opportunity has arisen for a compassionate, motivated, and experienced individual to join our dedicated SWAN Bereavement Team across the Northern Care Alliance NHS Foundation Trust.
Working in alignment with the SWAN model of care, you will provide discreet and empathetic support to individuals and families during the immediate stages of grief, regardless of age, cause, or place of death. This role involves educating, supporting, and empowering healthcare professionals to deliver practical and emotional care, including memory-making, guidance, and information before and after the death of a loved one.
Our team offers a compassionate presence and support to bereaved families, including those navigating the coronial process and attending inquests.
The service also supports tissue donation processes and contributes to the I-Orbit Project in collaboration with NHS Blood and Transplant, including screening and initial family approaches for potential corneal and tissue donation. You will receive comprehensive training in tissue donation processes from both NHS Blood and Transplant and the Bereavement Team.
This is not a counselling role, but one that provides a vital space for listening, understanding, and supporting people through the early stages of grief.
Main duties of the job
We’re looking for a compassionate, motivated, and experienced individual who can work collaboratively with a range of professionals and agencies in emotionally complex situations.
You will be a registered nurse with experience in palliative, end-of-life, or bereavement care, and possess excellent communication and interpersonal skills, with the ability to support families sensitively and professionally. You will demonstrate a commitment to compassionate care and continuous learning.
You will support tissue donation processes, including screening and making initial approaches to families regarding potential corneal and tissue donation, as well as providing related education and support to wards and departments.
The Bereavement Service is not a counselling service; however, it can refer or signpost families and carers to appropriate services when this level of intervention is identified. The role may also involve supporting families through the coronial process, including those required to attend an inquest.

- Completed Sage and Thyme communication skills training or equivalent
- Post registration study in End of Life or Bereavement

Knowledge, training and experience

- Experience in end of life care and bereavement support
- Understanding of and involvement in current end of life care, bereavement and tissue donation issues
- Evidence of relevant involvement in quality improvement initiatives
- Ability to travel to different geographical locations across the Trust footprint in line with service needs
Desirable
- Experience in providing multidisciplinary education
- Full UK driving license with willingness to use own car for work purposes
