We are looking for a Band 6 District Nursing Caseload Holder who would relish the challenge of working within primary care and caring for patients with complex needs whilst working towards an integrated healthcare structure. We are looking for candidate’s who are self-motivated, keen to work in a progressive division and to develop their skills and knowledge in supporting patients who are housebound with long term conditions and complex needs.
The service operates 24 hours per day, 365 days per year to cover our 8am-8pm 7 days a week model.
Main duties of the job
The post holder will have continuing responsibility for the management of a defined caseload. They will be responsible for the continuity of care and be the lead person for communication to identified stakeholders. The primary function of the role is to provide proactive, responsive services that address the anticipatory and maintenance care needs of the patients/clients within the defined caseload. This will be done via a holistic assessment of care needs and the provision of clinical care for patients/clients with acute and chronic highly complex needs.

The successful candidate should be able to work autonomously while demonstrating a team approach, and have strong clinical leadership firmly embedded within their role. We will also require excellent communication skills, clinical decision making skills and a problem solving approach. The successful candidate will be well supported, developed, and have the opportunity to work in an organisation that demonstrated inclusive and supportive leadership.
