Join Our Community Nursing Team
An exciting opportunity has arisen for an experienced and enthusiastic District Nurse to join our Community Nursing Service. We are looking for a motivated practitioner who is passionate about delivering high-quality care and supporting people to remain independent within their own homes and communities.
As a Band 7 District Nurse, you will manage a complex caseload, provide expert clinical assessment and intervention, and work collaboratively with multidisciplinary colleagues to ensure patients receive safe, effective and person-centred care. You will play a pivotal role in supporting patients with long-term conditions, frailty, palliative and end-of-life care needs, and those requiring complex clinical interventions within the community setting.
Key Responsibilities
- Deliver advanced clinical assessment, planning and evaluation of care for patients with complex healthcare needs.
- Manage and coordinate care to support individuals to remain safely at home where appropriate.
- Provide specialist district nursing expertise to patients, families and carers.
- Undertake complex wound management, palliative care, long-term condition management and other community nursing interventions.
- Work in partnership with GPs, Allied Health Professionals, Social Care and wider multidisciplinary teams.
- Contribute to service development, quality improvement initiatives and evidence-based practice.
- Support and supervise junior staff, students and healthcare support workers as required.
- Maintain high standards of clinical governance, patient safety and professional practice.
About You
To be successful in this role, you will:
- Be a Registered Nurse with current NMC registration.
- Hold a Specialist Practitioner Qualification in District Nursing.
- Have significant experience working within community nursing services.
- Demonstrate excellent clinical assessment and decision-making skills.
- Have experience managing patients with complex and unpredictable healthcare needs.
- Possess strong communication and interpersonal skills.
- Be committed to delivering person-centred care and continuous service improvement.
A full UK driving licence and access to a vehicle for work purposes may be essential for this post.
