We are seeking a motivated and compassionate District Nurse to join our community team. This is a rewarding opportunity to make a real difference by delivering high-quality care to patients in their own homes.

Essential Criteria:

  • District Nursing qualification (DN SPQ)

Key Responsibilities:

  • Assess patients in their homes, developing, implementing, and evaluating individualised care plans
  • Manage a caseload with the support of the Clinical Lead
  • Provide leadership and line management to a team of Registered Nurses and Healthcare Assistants
  • Work flexibly across teams to meet changing service demands and ensure continuity of care

Our core service operates 8:00am – 6:00pm, 7 days per week, and flexibility is essential to support the needs of the service.

This role requires a professional who demonstrates a positive attitude, compassion, resilience, and vigilance, ensuring safe and effective patient-centred care at all times.

Main duties of the job

What you will be doing

  • A District Nursing qualification (DN SPQ) is an essential criteria.
  • Assessing new patients for nursing care in their own homes, creating and evaluating care plans.
  • Managing a caseload with support of the Clinical Lead
  • Line Management and leadership of a team of Registered Nurses & HCAs.
  • Our core service hours are 08:00 to 18:00, 7 days per week.
