
The role is central to the co-ordination of both complex and non-complex discharges. The post will be based within the Patient Flow Hub, and have responsibility for overseeing non-complex and complex patient discharges within PTHB Community Hospitals. In the course of their duties will be responsibility for;
Facilitating and/or assist with effective, efficient and timely discharge of patients from hospital. Particular emphasis is placed upon quick turnaround of patients. This may include complex multi-disciplinary assessment signposting, liaising directly with patients and their families/ Care Transfer Coordinators/ MDT’s assisting in case conferences for patients with complex health and social care, palliative care or rehabilitation or housing needs. Giving families support and information.
Support the patient flow hub, as well as working closely with a range of professionals and external agencies within the hospital and in community services.
Work independently, following procedures and treatment plans, but under the direction of the patient flow hub, and within agreed boundaries.
Responsible for ongoing/complex planning in regards to process mapping patients journeys for modelling purposes.
Provide training on systems to new staff joining the team.

Work as part of the flow hub and work effectively with the wider multi-disciplinary team and third sector groups to facilitate patient discharges in a timely manner across Powys Supporting Ward staff and multi-disciplinary teams in facilitating effective, safe, and timely patient discharge.
