The post holder will be supported by more experienced nurses to enable them to develop a level of competence in order to work autonomously in providing specialist diabetes care. They will be required to support the CNS’s in the management of inpatients with diabetes at UHNM and they will also be responsible for managing their own caseload in the outpatient / Community setting and supporting MDT clinics, seeking advice and supervision from the established diabetes team where appropriate.

Main duties of the job

  • Through work based learning and development the post holder will develop specialist knowledge and skills.
  • To support the CNS’s in providing an evidence-based specialist diabetes nursing service which reflects current local and national guidelines.
  • To be accountable for their safe, effective clinical practice and to comply with the statutory NMC requirements.
  • Provide education and support to patients and their carers to facilitate self-management.

Provide assessment and evaluation of inpatient care needs and offer advice and support to ward based staff managing the inpatient with diabetes.

Support out-patient/ community services, MDT and nurse led clinics

Detailed job description and main responsibilities

The post-holder will play a key role in ensuring that the Trust provides high quality, person focused care, using current evidence-based practice. To also provide support, expert guidance and education to people living with diabetes, their carers/families and healthcare professionals involved in caring for people with diabetes.

The post-holder will be a core member of the Clinical Nurse Specialist team, working in all areas of diabetes and must have a strong teamwork ethos. They will be required to work autonomously and manage their own caseload, run nurse-led clinics and support other healthcare professionals in specialist MDT clinics, such as antenatal clinic, renal clinic, young person’s clinic, etc to meet the often-complex healthcare needs of people living with diabetes. In addition, they will be required to support and supervise junior members of the team.

The post will primarily be based at the University Hospital of North Midlands, County Site.  The post holder will be expected to contribute to the identification of the diabetes health care needs of the local health economy and work towards promoting and improving diabetes care both in the acute hospital setting and the wider community.

Education

Essential

  • Educated to degree level
  • Proficient in the use of Microsoft office: Word, Excel, access and Power-point
  • Portfolio of certified/accredited professional development activity undertaken in relevant areas
  • Experience and/or qualifications in the specific area

Desirable

  • Experience of working in a diabetes specialism
  • Non-medical prescribing qualification
