The post holder will provide multidisciplinary clinical staff with the support to effectively implement the principles and practice of clinical governance across all its component parts in a clinical setting, within a framework which uses information to guide reflection, leading to action and outcomes monitoring. This includes patient, family and public involvement, risk management, clinical audit, staffing and staff involvement, education and training and the use of information.

The post holder will undertake these functions under the line management of the Women’s Health Clinical Governance Manager, working in conjunction with the senior management, service leads and all staff in WHS.

Main duties of the job

  • Lead and support the management of clinical incidents, near misses and patient safety investigations, ensuring these are undertaken in accordance with Trust policy and the Patient Safety Incident Response Framework (PSIRF). Support on  maintaining the Women's Health risk register, identifying emerging risks and working collaboratively with clinical teams to develop effective mitigation plans. The post holder will also support compliance with national maternity and women's health initiatives, including Maternity Incentive Scheme (MIS) requirements, the Three-Year Delivery Plan, Perinatal Mortality Review Tool (PMRT), maternity safety improvement programmes, care bundles and regulatory standards. Support the development, review and implementation of clinical policies, guidelines and standard operating procedures, ensuring they remain evidence-based, compliant with national guidance. The post holder will also contribute to audit programmes, quality improvement initiatives, governance reporting and staff education to support the ongoing development of safe, effective and responsive services.

The post holder will undertake these functions under the line management of the Women’s Health Clinical Governance Manager, working in conjunction with the senior management, service leads and all staff in WHS.

  • Ensure that all clinical incidents and near misses are appropriately reported in accordance to trust policy.
  • To work with the senior team and other staff to develop appropriate action plans to minimise risk, including identifying resource implications, changes in clinical practice, administrative procedures, etc., required.
  • To ensure that all investigation processes within Maternity are conducted in accordance with Trust policy.
  • To undertake learning responses in line with the patient safety incident response framework, including write up of reports of such patient safety incident investigations and make recommendations for action as appropriate within agreed timescale, incorporating human factors.
  • To disseminate learning from clinical incidents including presenting findings of learning responses after clinical incidents undertaken at Trust wide groups.
  • To conduct regular reviews of previously reported incidents, etc, to evaluate action plans determine whether or not recommendations have been implemented.
  • Ensure that staff involved in incidents are provided with psychological support, debriefed and offered individual feedback including co­ ordination after action review sessions. Meet with women and families and any external agencies affected by adverse incidents.
  • To research best practice in clinical risk management, including learning from other organisations and clinical services.
  • Ensure that the divisional Risk register is kept up to date.
  • To be aware of complaints in relation to issues requiring investigation.
  • Support with the delivery of the national schemes (MIS, 3 year delivery plan, PMRT, Care bundles, CQC)

  • • Minimum 5 years post registration experience
  • • Minimum 3 years recent experience at Band 7
  • • Experience of working in a multidisciplinary environment with a wide range of individuals, at different levels

Education / Qualifications

Essential

  • • Registered Midwife
  • • Degree in Midwifery/health related studies or equivalent
  • • Mentorship or equivalent course
  • • Experience should include working at a senior level within and organisation/ report writing.
  • • Evidence of continuing education & professional development

Desirable

  • • (MSc level) or equivalent experience in one or more of the following- Governance systems and process (e.g. complaints management/ risk management /quality assurance systems/governance/Improvement/Leadership)
