We are looking for dynamic, passionate and motivated nurse with experience of Community Nursing to be a Team Leader within St Austell Community Nursing Team serving the St Austell Primary Care Network Area.
The successful candidate will work with the team to ensure provision of a holistic nursing assessment and high quality, safe, evidence based care to patients and carers with the aim of maintaining patients in their own homes. Our patients are at the centre of all that we do so we are looking for nurses who are calm, caring, compassionate clinicians with an interest in delivering the highest quality care and to support our patients to lead full and independent lives whilst promoting self management of their health conditions.
This will be an exciting and evolving job role within the Primary Care Network and explore new multi-disciplinary ways of integrated working. Therefore flexibility and a willingness to explore new exciting ways of working is a must and be adaptable accordingly.
This job is currently only open to people who already work for the NHS in Cornwall or the Isles of Scilly.

Main duties of the job
As an autonomous practitioner the post holder will be responsible for the assessment and treatment of patients with complex health and social care needs. They will manage a clinical caseload within the community nursing team with guidance from the Community Clinical Lead, Operational team manager, Community Matrons and Integrated Care Team Leads. Ensuring provision of a holistic nursing assessment and high quality, safe, evidence based care to patients and carers with the aim of maintaining patients in their own homes. This includes physical examination and detailed history taking, diagnosis of deterioration in pre-existing conditions and treatment planning.
The post holder will work in this adaptable and evolving role, focusing on safe, effective admission avoidance and supporting timely discharge from hospitals and other care settings. Implementing personalised care plans and collaborating with the whole community resource to facilitate ongoing care management.
To work in partnership with patients and carers and members of the multi-disciplinary team to offer support and advice and promote informed choice; maximising independence and optimising quality of life. This post holder will be required to provide clinical management and leadership to all staff within the team.
