Main duties of the job

Are you passionate about making a real difference to people living with frailty and complex long-term conditions? Join our Complex Care Nursing team and help enable people to live well at home and reduces hospital admissions.
Working across a designated Primary Care Network, you will support a caseload alongside experienced Band 6 and Band 7 Complex Care Nurses. You'll undertake routine assessments, patient reviews and monitoring, supporting people with conditions such as Heart Failure, COPD and diabetes.
Key responsibilities include phlebotomy, blood pressure and weight monitoring, Continuous Glucose Monitoring (CGM) set-up and review, foot and skin assessments, administering medications including insulin, and providing education to promote self-management and independence.
This is an excellent opportunity to develop your clinical skills within a supportive team, with access to training and development opportunities that will help you build confidence in managing patients with increasingly complex needs.
