Complex Care Lead

Be the lead point of contact for MK health and social care professionals in need of transforming care support with complex cases relating to mental health risk of admissions, care and treatment progress during admissions and advice during discharge planning.

Main duties of the job

Coordinate and manage the Dynamic Support Register for the MK area.

Facilitate and chair Care and Treatment Reviews when indicated - both community and Inpatient CTRs.

Undertake Quality Oversight Visits to patients in MH hospitals every 8 x weeks to monitor care and treatment during the admissions.

Communication and Relationship Skills

The post holder will be responsible for holding and communicating sensitive and confidential patient information.

To work in collaboration with commissioners, BLMK wide health, social care, independent and voluntary services.

• Planning and Organisational Skills The post holder will lead the coordination and chairing of Care and Treatment Reviews (CTR) and Local Area Emergency Protocols (LAEPs) for people with LD / ASD in Milton Keynes.  This will be achieved through a Dynamic Support Register to support the reduction of admissions and ensure timely plans are in place for discharge from hospital in line with the CTR Policy. To work in collaboration with Children Services to plan for well supported transitions into adult services, for young people with LD / ASD and complex needs. To monitor out of area hospital placements and carry out quality oversight visits in line with the national guidance for people with a learning disability and/or are autistic. . To manage competing priorities in a busy and changing environment.

Clinical Skills

• The post holder will provide clinical expertise to the commissioning process to support strategic developments for people with a learning disability and/or who are autistic.

Lead relevant operational meetings, work streams and associated projects/meetings.

To liaise with counterparts in Luton and Bedfordshire.

• To contribute to discussions regarding  S117 eligibility and the proposed care and support for people with a learning disability and/or people who are autistic ensuring options are both of good quality and demonstrate best value. . To work collaboratively with people who are being supported within in-patient settings and their respective care team to ensure effective and timely communication that supports timely and appropriate discharges.
