Job overview

As an enthusiastic registered nurse with community experience and with current NMC registration and Mentorship Training qualifications, you will have good communication and computer skills and experience of working in a customer focused environment. You will be excited about learning and willing to participate in further training and development.

Enjoying collaboration, you will work with the Community Nursing Team Leader, Clinical Leads and the Community Nursing Manager and will provide clinical support and assistance to the service. This role will improve and maintain standards of patient care.

Your role will be responsible for leading wound / pressure care management and be an expert resource for community staff across the Reading Locality. The post holder will ensure the delivery of evidence-based care in relation to all aspects of Tissue Viability.

Main duties of the job

  • Make and receive referrals for patients with compromised tissue viability, from nurses, general practitioners, consultants and other health professional.
  • Assess, plan, implement and evaluate patient care utilising advanced knowledge and skills and research and developments in tissue viability.
  • Be available to offer expert advice on all aspects of wound care, providing clinical advice and support to staff in meeting the standards and protocols in delivery of care.
  • Assess current treatment practices, maximise cost effectiveness whilst improving outcomes.
  • Provide information and advice to healthcare professionals about tissue viability using enhanced clinical judgement. Act as an information resource on tissue viability issues.
  • To support and deliver the provision of education and training relating to tissue viability to all levels of staff and disciplines.
  • Support and contribute to the development of programmes of care to improve clinical outcome in the prevention and treatment of pressure ulcers, leg ulcers and non-healing and complex wounds.
