<p>As a Band 5 Community Staff Nurse, you will join our valued Swanley and Rural PCN Community Nursing Team in North Kent. This is a rewarding opportunity to deliver high-quality nursing care where it matters most, within patients\' own homes and local communities. You will <span class="single_desc_int_gb_word">support people with a wide range of healthcare needs, helping them manage long-term conditions, recover from illness and maintain their independence while avoiding unnecessary hospital admissions</span>. Whether you are newly qualified or looking to build your career in community nursing, you will join a <span class="single_desc_int_gb_word">supportive, welcoming team that is passionate about delivering excellent patient-centred care</span>.
