An exciting opportunity has arisen for an experienced Band 5 Community Staff Nurse to join our Barnet Integrated Community Service by providing a high quality service to our housebound patients by facilitating earlier hospital discharges and preventing unnecessary hospital admissions.
As a Band 5 Community Staff Nurse you would be responsible for providing a safe, personalised and effective care to patients who require community nursing care by working alongside the community nursing team to deliver clinical and administrative duties.

Main duties of the job
- Provide high quality, patient centred nursing care to housebound patients within the community, supporting them to maintain independence and wellbeing.
- Undertake holistic assessments, implement care plans, and deliver nursing interventions in line with individual patient needs.
- Work as part of the Integrated Community Nursing Service to support safe hospital discharge, case management, and admission avoidance.
- Collaborate effectively with the multidisciplinary team, including Acute Hospitals, GPs, Hospices, and other community services to ensure coordinated care.
- Communicate clearly and compassionately with patients, carers, and colleagues, demonstrating strong interpersonal skills.
- Maintain accurate, timely, and comprehensive clinical records using electronic systems.
- Adapt to changing service demands and work patterns, remaining calm and professional in a fast paced environment.
- Participate in team meetings, supervision, and ongoing training to support your development as a Band 5 Community Nurse.
The Integrated Community Nursing Services has a key focus on facilitating discharges, case management and reducing admission to hospital. The service works closely with Acute Hospitals, Hospice, including GPs. The service provides care to patients in the community setting. You will be well supported by our current team and provision is also made for your continued professional development.

Experience
Essential
- Recent clinical nursing experience negotiating and liaising within a multi-disciplinary team
- Working with people with complex health needs
- Supporting students and other learners e.g. HCAs teaching others in the practice environment
- Experience of undertaking general assessment by utilising evidence-based care.
- Previous experience in the Nursing process, care planning and fluent in record keeping
- Experience of providing holistic care that involves the multidisciplinary team
Desirable
- Experience of training other students.
- Participation in audit; research projects; service development
- Post registration experience in a NHS community environment

- Ability to work in a team and be a proactive member and able to engage in change process in the team.
- Ability to organise and participate in weekly work allocation and be able to prioritise and delegate effectively.

- Knowledge of out of hospital care and the role of the Community Nurse
- Ability to listen effectively and ability to give constructive feedback to clients and colleagues
- Knowledge of how Equal Opportunities can be implemented in practice
- Knowledge of reasonable response to service user needs and prioritising and dealing with those needs with a required urgency
- Awareness of the management and use of data in the NHS setting
- Knowledge of management of long term condition e.g. Diabetes and an understanding of the concept of self- management.
- Knowledge of evidence based practice.
- Knowledge of general wound care
- Knowledge of infection control
